Healthcare-associated infections (HAIs) are a major worldwide public-health problem, but less data are available on the long-term trends of HAIs and antimicrobial use in Eastern China. This study describes the prevalence and long-term trends of HAIs and antimicrobial use in a tertiary care teaching hospital in Hefei, Anhui, China from 2010 to 2017 based on annual point-prevalence surveys. A total of 12 505 inpatients were included; 600 HAIs were recorded in 533 patients, with an overall prevalence of 4.26% and a frequency of 4.80%. No evidence was found for an increasing or decreasing trend in prevalence of HAI over 8 years (trend χ2 = 2.15, P = 0.143). However, significant differences in prevalence of HAI were evident between the surveys (χ2 = 21.14, P < 0.001). The intensive care unit had the highest frequency of HAIs (24.36%) and respiratory tract infections accounted for 62.50% of all cases; Escherichia coli was the most common pathogen (16.67%). A 44.13% prevalence of antimicrobial use with a gradually decreasing trend over time was recorded. More attention should be paid to potential high-risk clinical departments and HAI types with further enhancement of rational antimicrobial use.
We examined the in vitro developmental competence of parthenogenetic activation (PA) oocytes activated by an electric pulse (EP) and treated with various concentrations of AZD5438 for 4 h. Treatment with 10 µM AZD5438 for 4 h significantly improved the blastocyst formation rate of PA oocytes in comparison with 0, 20, or 50 µM AZD5438 treatment (46.4% vs. 34.5%, 32.3%, and 24.0%, respectively; P < 0.05). The blastocyst formation rate was higher in the group treated with AZD5438 for 4 h than in the groups treated with AZD5438 for 2 or 6 h (42.8% vs. 38.6% and 37.2%, respectively; P > 0.05). Furthermore, 66.67% of blastocysts derived from these AZD5438-treated PA oocytes had a diploid karyotype. The blastocyst formation rate of PA and somatic cell nuclear transfer (SCNT) embryos was similar between oocytes activated by an EP and treated with 2 mM 6-dimethylaminopurine for 4 h and those activated by an EP and treated with 10 µM AZD5438 for 4 h (11.11% vs. 13.40%, P > 0.05). In addition, the level of maturation-promoting factor (MPF) was significantly decreased in oocytes activated by an EP and treated with 10 µM AZD5438 for 4 h. Finally, the mRNA expression levels of apoptosis-related genes (Bax and Bcl-2) and pluripotency-related genes (Oct4, Nanog, and Sox2) were checked by RT-PCR; however, there were no differences between the AZD5438-treated and non-treated control groups. Our results demonstrate that porcine oocyte activation via an EP in combination with AZD5438 treatment can lead to a high blastocyst formation rate in PA and SCNT experiments.
Gradual fault detection is always an important issue in integrated navigation systems, and the gradual fault is the most difficult fault to detect. To detect gradual faults in a timely and precise manner in integrated navigation systems, the statistical concepts of the normalised residual mean and the sum of absolute residuals are introduced according to the characteristics of gradual system failure in this paper. The applicability of the improved residual χ2 detection method is discussed. Then, the gradual fault detection program based on the improved residual χ2 detection method is designed with the criterion of normalised residual mean and the sum of absolute residual. The simulation results and vehicle tests show that: 1) The residual of the failed sub-system can be calculated accurately with the improved residual χ2 detection method, which has strong applicability in gradual fault detection; 2) The gradual fault can be detected in a short time by using the normalised residual mean and the sum of absolute residual.
The present study aimed to compare the effects of a general dietary intervention and an intervention with low glycaemic load (GL) on glycaemic control, blood lipid metabolism and pregnancy outcomes in women with gestational diabetes mellitus.
Participants were randomly assigned to two groups, receiving either an individualized general dietary intervention (Control group) or an intensive low-GL intervention (Low-GL group) every two weeks, from 24–26 weeks of gestation to delivery.

Ninety-five women with gestational diabetes mellitus were enrolled from June 2008 to July 2009.
After the intervention, both groups significantly decreased their dietary intakes of energy, fat and carbohydrate. The Low-GL group had significantly lower values for GL (122 v. 136) and glycaemic index (50 v. 54) but greater dietary fibre intake (33 v. 29 g/d) than did the Control group (all P<0·01). Significantly greater decreases in fasting plasma glucose (−0·33 v. −0·02 mmol/l, P<0·01) and 2 h postprandial glucose (−2·98 v. −2·51 mmol/l, P<0·01), significantly lower increases in total cholesterol (0·12 v. 0·23 mmol/l) and TAG (0·41 v. 0·56 mmol/l) and a significantly lower decrease in HDL cholesterol (−0·01 v. −0·11 mmol/l) were also observed in the Low-GL group compared with the Control group (all P<0·05). There were no significant differences in body weight gain, birth weight or other maternal–fetal perinatal outcomes between the two groups.
The low-GL targeted dietary intervention outperformed the general dietary intervention in glycaemic control and the improvement of blood lipid levels in women with gestational diabetes mellitus.
A method to design the composite insulation structures in pulsed power systems is proposed in this paper. The theoretical bases for this method include the Weibull statistical distribution and the empirical insulation formula. A uniform formula to describe the reliability (R) for different insulation media such as solid, liquid, gas, vacuum, and vacuum surface is derived. The dependence curves of the normalized applied field on R are also obtained. These curves show that the normalized applied field decreases rapidly as R increases but the declining rates corresponding to different insulation media are different. In addition, if R is required to be higher than a given level, the normalized applied field should be smaller than a certain value. In practical design, the common range of the applied fields for different insulation media should be chosen to meet a global reliability requirement. In the end, the proposed method is demonstrated with a specific coaxial high-voltage vacuum insulator.
Neurosyphilis (NS) may present with neuropsychiatric disorders characterized by cognitive impairment, personality disorders, and confusion, among others. Very few studies have focused on neuropsychiatric disorders secondary to NS in elderly people.
A retrospective chart review was performed to characterize the psychiatric findings, clinical signs and symptoms, laboratory findings, and brain magnetic resonance imaging results of ten elderly inpatients with NS.
In these ten patients, the most common presenting symptoms included a wide variety of psychiatric manifestations. The serum rapid plasma regain (RPR) and Treponema pallidum particle agglutination assay (TPPA) of the ten patients were positive, with positive CSF TPPA and RPR rates of 100% and 60%, respectively. In addition, 90% of the patients demonstrated abnormal imaging, including cerebral atrophy, infarct ischemic stroke, and hydrocephalus.
Our findings support the importance of serological tests for syphilis as a routine component of the evaluation of patients with clinically evident neurological or psychiatric symptoms. If the serology is positive, all of the patients should be examined with a lumbar puncture. Moreover, psychiatric illnesses secondary to NS in the elderly also deserve medical attention.
The phosphatocopids Hesslandona necopina Müller, 1964 and Hesslandona longispinosa (Kozur, 1974) new combination, recovered from the Upper Cambrian in western Hunan, South China, are described. The ontogenetic stages of H. necopina are revised, with newly defined second and third ontogenetic stages. The second stage is characterized by a bipartite mandibular limb stem consisting of separate coxa and basipod. The third stage is characterized by the partial fusion of the mandibular coxa and basipod. These two parts may become completely fused possibly in a later ontogenetic stage. Vestrogothia longispinosa Kozur, 1974 is reassigned to Hesslandona because of the presence of an interdorsum and a relatively narrow doublure. The new data reported here, plus earlier reports of phosphatocopids from the same section and horizon in western Hunan since 2005, are included in an updated phylogenetic analysis of the Phosphatocopida. Autapomorphies that define several monophyletic groups (e.g., the Crustacea sensu lato, Labrophora, Eucrustacea, Phosphatocopida, Euphosphatocopida, Vestrogothiina, Hesslandonina, and Dorsospinata) are discussed. The present analysis confirms results from earlier phylogenetic analyses in showing the Phosphatocopida-Eucrustacea sister-taxon relationship, but differs from them in supporting the paraphyly of the Hesslandonidae and the monophyly of the Vestrogothiidae.
This paper presents an approach for decoding the pressure information exerted over a piece of fabric by means of resistive sensing. The proposed sensor includes a distributed resistive grids constructed by two systems of orthogonally contacted electrical conductive yarns, with no external sensing element to be attached on the fabric. Since the conductive yarns serve as the sensing and wiring elements simultaneously, this design simplifies the fabrication process, reduces the cost and makes the production of large area flexible pressure sensor possible. The location of the pressure applied on the fabric can be identified by detecting the position where the change of the resistances occurs between two embroidered yarns. Meanwhile, the magnitude of the pressure can be acquired by measuring the variations of the resistance. In order to eliminate the “crosstalk” effect between adjoining fibers, the yarns were separately wired on the fabric surface.
CdS nanocrystals embedded in sodium borosilicate glass were synthesized through sol-gel process. The CdS nanocrystals were usually 10 to 20 nm in size. The microstructure of CdS nanocrystals was determined to be of the hexagonal phase. The morphology and microstructure of the glass were examined using diverse techniques including scanning-probe microscopy (SPM), x-ray photoelectron spectroscopy (XPS), transmission electron microscopy (TEM), scanning transmission electron microscopy (STEM), energy dispersion x-ray spectra (EDAX), and high-resolution TEM (HRTEM). The linear optical absorption spectrum of the glass showed a blue shift as a result of quantum-size effect. Furthermore, the third-order optical nonlinearities of the glass were studied by Z-scan technique at a wavelength of 770 nm. The results showed that the third-order optical nonlinear refractive index γ, absorption coefficient β, and susceptibility χ(3) were determined to be −2.16 × 10−16 m2/W, 6.32 × 10−11 m/W, and 1.20 × 10−10 esu, respectively, which were greater than those reported previously for CdS nanocrystals embedded in different matrices.
The strategies of repair of tetralogy of Fallot change with the age of patients. In children older than 4 years and adults, the optimal strategy may be to use different method of reconstruction of the right ventricular outflow tract from those followed in younger children, so as to avoid, or reduce, the pulmonary insufficiency that is increasingly known to compromise right ventricular function.
From April, 2001, through May, 2008, we undertook complete repair in 312 patients, 180 male and 132 female, with a mean age of 11.3 years ±0.4 years, and a range from 4 to 48 years, with typical clinical and morphological features of tetralogy of Fallot, including 42 patients with the ventriculo-arterial connection of double outlet right ventricle. The operation was performed under moderate hypothermia using blood cardioplegia. The ventricular septal defect was closed with a Dacron patch. When it was considered necessary to resect the musculature within the right ventricular outflow tract, or perform pulmonary valvotomy, we sought to preserve the function of the pulmonary valve by protecting as far as possible the native leaflets, or creating a folded monocusp of autologous pericardium.
The repair was achieved completely through right atrium in 192, through the right ventricular outflow tract in 83, and through the right atrium, the outflow tract, and the pulmonary trunk in 36 patients. A transjunctional patch was inserted in 169 patients, non-valved in all but 9. There were no differences regarding the periods of aortic cross-clamping or cardiopulmonary bypass. Of the patients, 5 died (1.6%), with no influence noted for the transjunctional patch. Of those having a non-valved patch inserted, three-tenths had pulmonary regurgitation of various degree, while those having a valved patch had minimal pulmonary insufficiency and good right ventricular function postoperatively, this being maintained after follow-up of 8 to 24-months.
Based on our experience, we suggest that the current strategy of repair of tetralogy of Fallot in older children and adults should be based on minimizing the insertion of transjunctional patches, this being indicated only in those with very small ventriculo-pulmonary junctions. If such a patch is necessary, then steps should be taken to preserve the function of the pulmonary valve.
The microsatellite, or short sequence repeat (SSR), is a powerful genetic marker, useful in many areas of fish genetics and breeding. Polymorphic microsatellite loci have been frequently applied to the analysis of genetic diversity, population genetic structure, and genomic mapping. These co-dominant markers have also been applied to the classification and systematics, parentage identification, germplasm conservation, and breeding programme of food fish.
Benign chondroma affecting the soft tissues is uncommon. This tumour is particularly rare in the neck. We present the first case of soft tissue chondroma arising in the parapharyngeal space of a 20-year-old man with a two-year history of a gradual sensation of a swelling in the pharynx with increasing snoring. The CT and pathological features of this lesion will be discussed. We suggest that a peroral approach should be considered for benign parapharyngeal tumour which has been demonstrated on imaging to be in the anterior parapharyngeal space, especially for tumours that are medial to the pterygoid muscles even though the tumour is a large one.
Radar, aerial netting and ground sampling were used to study the autumn migration of Nilaparvata lugens (Stål) in Jiangsu Province in east central China. Emigration of macropterous adults increased from late August until late September as the main rice crop matured and was harvested. In early and mid September, the resulting windborne migrations carried the planthoppers mainly towards the west, although the migration directions ranged (within the western sector) from south to north. By late September, however, displacements were predominantly to the south-west on the then prevailing north-east monsoon winds: migration was particularly rapid when the north-easterlies were reinforced by typhoons. Although in late September such movements to more southerly latitudes are essential for the survival of the planthoppers' progeny, we found no definite evidence for preferential emigration on winds blowing towards the south. There were, however, indications that when winds towards the north occurred, the duration of migratory flight was curtailed. Irrespective of any possible preference for migration on northerlies, a large proportion of the N. lugens population would normally be carried in an adaptive southwards direction, because the advent of the north-east monsoon occurs at a time when the number of flight-ready planthoppers approaches its peak.
