Antimicrobial-product package inserts and insufficient staffing impede routine carbapenem monitoring in the inpatient setting in Japan. The collaboration between antimicrobial stewardship teams and clinical pharmacists was associated with a sustained improvement in carbapenem dosing optimization. Our findings could be of use to countries with inadequate monitoring of carbapenem antimicrobial use.

The detection rate of multidrug-resistant Pseudomonas aeruginosa in patients admitted to 2 wards and the intensive care unit decreased from 20.3% (129 of 636 isolates) to 4.2% (37 of 889 isolates) after the start of disinfection of hand washing sinks using alkyl diaminoethylglycine hydrochloride.

We study passive scalar fluctuations convected by statistically stationary homogeneous isotropic turbulence under a uniform mean scalar gradient. In order to elucidate the parameter dependence of small-scale statistics of scalar fluctuations, we conduct direct numerical simulations of passive scalar turbulence with 59 different combinations of Reynolds number and Schmidt number. For all the cases, we compute time-average statistics of various quantities, which include the scalar derivative skewness and flatness, the ratio of parallel-to-perpendicular scalar-gradient variances, and the anisotropy parameter recently proposed (Hill, Phys. Rev. Fluids, vol. 2, 2017, 094601). Notably, the degree of small-scale anisotropy of passive scalar fluctuation is characterised by a universal function of the Péclet number $Pe_{\unicode[STIX]{x1D706}_{\unicode[STIX]{x1D703}}}=u^{\prime }\unicode[STIX]{x1D706}_{\unicode[STIX]{x1D703}}/\unicode[STIX]{x1D705}$, where $u^{\prime }$ is the root mean square velocity, $\unicode[STIX]{x1D706}_{\unicode[STIX]{x1D703}}$ the Taylor microscale of scalar fluctuation, $\unicode[STIX]{x1D705}$ the mass diffusivity. In the definition of the Péclet number, the use of $\unicode[STIX]{x1D706}_{\unicode[STIX]{x1D703}}$, rather than the Taylor microscale of velocity fluctuation, is key to collapsing the data of different Reynolds and Schmidt numbers. When the Péclet number is low, large-scale anisotropic scalar structures emerge irrespective of the Reynolds number. These structures are elongated along the direction of the uniform mean scalar gradient, and their size is significantly larger than the integral length scale of velocity fluctuation.

We are interested in standing waves of a modified Schrödinger equation coupled with the Chern–Simons gauge theory. By applying a constraint minimization of Nehari-Pohozaev type, we prove the existence of radial ground state solutions. We also investigate the nonexistence for nontrivial solutions.

We present the water column profiles (surface to 2000 m depth) for dissolved inorganic radiocarbon (14CDIC) from 2 stations in the Kuroshio region including the Kuroshio large meander (LM) of 2004–2005. Surprisingly, the Δ14CDIC value varied up to 125‰ in the intermediate layer, especially near 600 m depth. In addition, the Δ14CDIC value was approximately − 150‰ at 200 m depth at the northern station of Kuroshio in August 2005. This value is ∼100‰ less than other Δ14CDIC values for the same depth. In comparison, the Δ14CDIC water column profiles for the southern station of Kuroshio and GEOSECS station 224 decrease down to 600 m depth and were similar below 600 m depth. Our results suggest that strong upwelling associated with the Kuroshio LM has a powerful influence on the Δ14CDIC water column profiles in the study region.

Our preliminary clinical trial showed that consumption of cooked rice of a Japanese common cultivar Yukihikari improved atopic dermatitis associated with a suspected rice allergy, although the underlying mechanisms remain unclear. We hypothesised that the ameliorating effect of Yukihikari on atopic dermatitis is associated with the gut microbiota. BALB/c mice were fed a synthetic diet supplemented with uncooked and polished white rice powder prepared from one of four different cultivars: Yukihikari, rice A (common rice), rice B (brewery rice) and rice C (waxy rice). Denaturing gradient gel electrophoresis of PCR-amplified 16S rRNA gene fragments showed that the composition of faecal microbiota was different between mice fed Yukihikari and those fed rice A. Analysis of the 16S rRNA clone library and species-specific real-time PCR showed that the abundance of Akkermansia muciniphila, a mucin degrader, tended to be lower in mice fed Yukihikari. The incidence of allergic diarrhoea induced by oral administration of ovalbumin in systemically immunised mice was lower in mice fed Yukihikari, albeit with no difference in serum antibodies specific to ovalbumin. In a separate experiment, serum antibody levels specific to orally administered ovalbumin were lower in mice fed Yukihikari. Additionally, the translocation of horseradish peroxidase in isolated segments of ileum and colon tended to be lower in mice fed Yukihikari, suggesting a reduction in gut permeability in mice fed Yukihikari. These data indicate that changes in the gut microbiota of mice fed Yukihikari could be advantageous in the prevention of food allergy.

In this paper, we study a symmetry-breaking phenomenon of a least-energy solution to a nonlinear Schrödinger equation under suitable assumptions on V(x), where λ > 1, p > 2 and χA is the characteristic function of the set A = [−(l + 2), −l] ∪ [l,l + 2] with l > 0. We also study asymptotic profiles of least-energy solutions for the singularly perturbed problem for small ε > 0.
