The appointment of an Ad Hoc Reform Council, or Rinkyoshin, on 21 August 1984 was a logical culmination to a lengthy period of concern in Japan over a set of widely perceived educational problems and the future prospects for Japanese education. The charge given to the council by Prime Minister Yasuhiro Nakasone was clear: “to consider basic strategies for necessary reforms… so as to secure such education as will be compatible with the social changes and cultural developments of our country.” The prime minister went on to remind council members that “if our nation is to build up a society that is full of vitality and creativity as well as relevant to the 21st century, it is a matter of great urgency to design necessary reforms.”
In 1966, Henry K. Beecher published an article entitled “Ethics and Clinical Research” in the New England Journal of Medicine, which cited examples of ethically problematic human research. His influential paper drew attention to common moral problems such as inadequate attention to informed consent, risks, and efforts to provide ethical justification. Beecher’s paper provoked significant advancements in human research policies and practices. In this paper, we use an approach modeled after Beecher’s 1966 paper to show that moral problems with animal research are similar to the problems Beecher described for human research. We describe cases that illustrate ethical deficiencies in the conduct of animal research, including inattention to the issue of consent or assent, incomplete surveys of the harms caused by specific protocols, inequitable burdens on research subjects in the absence of benefits to them, and insufficient efforts to provide ethical justification. We provide a set of recommendations to begin to address these deficits.
This study aims to assess current practices of Canadian physicians providing botulinum toxin-A (BoNT-A) treatments for children with hypertonia and to contrast these with international “best practice” recommendations, in order to identify practice variability and opportunities for knowledge translation.
Thirteen Canadian physicians assembled to develop and analyze results of a cross-sectional electronic survey, sent to 50 physicians across Canada.
Seventy-eight percent (39/50) of physicians completed the survey. The most frequently identified assessment tools were Gross Motor Function Classification System, Modified Tardieu Scale and neurological examination. Goal-setting tools were infrequently utilized. Common indications for BoNT-A injections and the muscles injected were identified. Significant variability was identified in using BoNT-A for hip displacement associated with hypertonia. The most frequent adverse event reported was localized weakness; 54% reporting this “occasionally“ and 15% “frequently”. Generalized weakness, fatigue, ptosis, diplopia, dysphagia, aspiration, respiratory distress, dysphonia and urinary incontinence were reported rarely or never. For dosage, 52% identified 16 Units/kg body weight of Botox® as maximum. A majority (64%) reported a maximum 400 Units for injection at one time. For localization, electrical stimulation and ultrasound were used infrequently (38% and 19% respectively). Distraction was the most frequently used pain-management technique (64%).
Canadian physicians generally adhere to international best practices when using BoNT-A to treat paediatric hypertonia. Two knowledge-translation opportunities were identified: use of individualized goal setting prior to BoNT-A and enhancing localization techniques. Physicians reported a good safety profile of BoNT-A in children.
The structures of aluminum nitride and aluminum oxide powders have been modified by explosive shock treatment at pressures from 4 to 27 GPa. Shockinduced dislocations in AlN were apparently responsible for enhancement in hot pressing behavior. In Al2O3 an increase 3 to 4 times in densification rates after shocking resulted-from rapid sintering of aggregates in the powder.
