There are numerous challenges pertaining to epilepsy care across Ontario, including Epilepsy Monitoring Unit (EMU) bed pressures, surgical access and community supports. We sampled the current clinical, community and operational state of Ontario epilepsy centres and community epilepsy agencies post COVID-19 pandemic. A 44-item survey was distributed to all 11 district and regional adult and paediatric Ontario epilepsy centres. Qualitative responses were collected from community epilepsy agencies. Results revealed ongoing gaps in epilepsy care across Ontario, with EMU bed pressures and labour shortages being limiting factors. A clinical network advising the Ontario Ministry of Health will improve access to epilepsy care.

We investigated whether people’s risk taking tendency established in one domain (gains or losses) carries over to the other domain. Participants played a game in which they made repeated decisions between a fixed payoff and a risky option, where the outcome of the risky option depended on whether they had responded correctly on a difficult perceptual-memory task. In some trials, participants played to gain points; on others, they played to avoid losing points. In two studies, we observed the following pattern of results. 1) Participants risked less on gain trials than on loss trials. 2) This difference in risk taking persisted (carried over) when the domain changed from gains to losses and vice versa (with the effect of experiencing losses first being stronger than the effect of experiencing gains first). 3) There was no analogous carryover effect on responses to a delay discounting measure, but there was a carryover effect on responses on a risk attitude measure. We compare these results with those from other recent studies and discuss various ways of explaining them.

Since 1997, execution in China has been increasingly performed by lethal injection. The current criteria for determination of death for execution by lethal injection (cessation of heartbeat, cessation of respiration, and dilated pupils) neither conform to current medical science nor to any standard of medical ethics. In practice, death is pronounced in China within tens of seconds after starting the lethal injection. At this stage, however, neither the common criteria for cardiopulmonary death (irreversible cessation of heartbeat and breathing) nor that of brain death (irreversible cessation of brain functions) have been met. To declare a still-living person dead is incompatible with human dignity, regardless of the processes following death pronouncement. This ethical concern is further aggravated if organs are procured from the prisoners. Analysis of postmortem blood thiopental level data from the United States indicates that thiopental, as used, may not provide sufficient surgical anesthesia. The dose of thiopental used in China is kept secret. It cannot be excluded that some of the organ explantation surgeries on prisoners subjected to lethal injection are performed under insufficient anesthesia in China. In such cases, the inmate may potentially experience asphyxiation and pain. Yet this can be easily overlooked by the medical professionals performing the explantation surgery because pancuronium prevents muscle responses to pain, resulting in an extremely inhumane situation. We call for an immediate revision of the death determination criteria in execution by lethal injection in China. Biological death must be ensured before death pronouncement, regardless of whether organ procurement is involved or not.

Functionally graded materials (FGMs) in which the elemental composition intentionally varies with position can be fabricated using directed energy deposition additive manufacturing (AM). This work examines an FGM that is linearly graded from V to Invar 36 (64 wt% Fe, 36 wt% Ni). This FGM cracked during fabrication, indicating the formation of detrimental phases. The microstructure, composition, phases, and microhardness of the gradient zone were analyzed experimentally. The phase composition as a function of chemistry was predicted through thermodynamic calculations. It was determined that a significant amount of the intermetallic σ-FeV phase formed within the gradient zone. When the σ phase constituted the majority phase, catastrophic cracking occurred. The approach presented illustrates the suitability of using equilibrium thermodynamic calculations for the prediction of phase formation in FGMs made by AM despite the nonequilibrium conditions in AM, providing a route for the computationally informed design of FGMs.

A psychiatric examination was conducted on 810 community dwelling subjects previously given a diagnosis derived from the Diagnostic Interview Schedule (DIS). The agreement in allocating subjects to a particular disorder was never high enough to encourage the confident replacement of a psychiatric diagnosis with a DIS diagnosis.

Coherent control of molecular processes provides a means of controlling the dynamics of molecules, and of molecular processes, via laser-induced quantum interference. We briefly review this approach, provide relevant references, and highlight recent advances in the control of molecular processes.

In this paper we consider the family of rational maps of the complex plane given by \[z^2+\frac{\lambda}{z^2}\] where $\lambda$ is a complex parameter. We regard this family as a singular perturbation of the simple function $z^2$. We show that, in any neighborhood of the origin in the parameter plane, there are infinitely many open sets of parameters for which the Julia sets of the corresponding maps are Sierpinski curves. Hence all of these Julia sets are homeomorphic. However, we also show that parameters corresponding to different open sets have dynamics that are not conjugate.

The propagation of cosmological ionisation fronts (I-fronts) during reionisation is strongly influenced by small-scale structure. Here we summarise our recent attempts to understand the effect of this small-scale structure. We present high resolution cosmological N-body simulations at high-z ($z>6$) which resolve a wide range of halo mass, from mini-halos to clusters of large, rare halos. We also study how mini-halos affect I-fronts, through simulations of mini-halo photo-evaporation including numerical gas dynamics with radiative transfer. Furthermore, we modify the I-front propagation equations to account for evolving small-scale structure, and incorporate these results into a semi-analytical reionisation model. When intergalactic medium clumping and mini-halo clustering around sources are included, small-scale structure affects reionisation by slowing it down and extending it in time. This helps to explain the observations of the Wilkinson Microwave Anisotropy Probe, which imply an early and extended reionisation epoch. We also study how source clustering affects the evolution and size of H II regions, finding, in agreement with simulations, that H II regions usually expand, and rarely shrink. Hence, “relic H II regions” are an exception, rather than the rule. When the suppression of small-mass sources in already-ionised regions by Jeans-mass filtering is accounted for, H II regions are smaller, delaying overlap. We also present a new numerical method for radiative transfer which is fast, efficient and easily coupled to hydrodynamics and N-body codes, along with sample tests and applications.
