This study aimed to investigate the effects of esketamine (Esk) combined with dexmedetomidine (Dex) on postoperative delirium (POD) and quality of recovery (QoR) in elderly patients undergoing thoracoscopic radical lung cancer surgery.
Methods
In this prospective, randomized, and controlled study, 172 elderly patients undergoing thoracoscopic radical lung cancer surgery were divided into two groups: the Esk + Dex group (n = 86) and the Dex group a (n = 86). The primary outcome was the incidence of POD within 7 days after surgery and the overall Quality of Recovery−15 (QoR − 15) scores within 3 days after surgery. Secondary outcomes included postoperative adverse reactions, extubation time, PACU stay, and hospitalization time. Serum levels of IL-6, IL-10, S100β protein, NSE, CD3+, CD4+, and CD8+ were detected from T0 to T5.
Results
Compared with the Dex group, the incidence of POD in the Esk + Dex group was significantly lower at 7 days after surgery (14.6% vs 30.9%; P = 0.013). The QoR − 15 score was significantly increased 3 days after surgery (P < 0.01). Levels of IL-6 and CD8+ were significantly decreased, and IL − 10 levels were significantly increased at T1-T2 (P < 0.05). At T1-T4, NSE levels were significantly decreased, while CD3+ and CD4+/CD8+ values were significantly increased (P < 0.01). At T1-T5, serum S100β protein concentration decreased significantly, and CD4+ value increased significantly (P < 0.01). The incidence of nausea/vomiting and hyperalgesia decreased significantly 48 hours after surgery (P < 0.01). The duration of extubation, PACU stay, and postoperative hospitalization were significantly shortened.
Conclusions
Esketamine combined with dexmedetomidine can significantly reduce the POD incidence and improve the QoR in patients undergoing thoracoscopic radical lung cancer surgery, which may be related to the improvement of cellular immune function.
This study compared survival outcomes between intensive care unit (ICU) patients receiving enteral nutrition (EN) and parenteral nutrition (PN) with vasopressor support, explored risk factors affecting clinical outcomes and established an evaluation model. Data from 1046 ICU patients receiving vasopressor therapy within 24 h from 2008 to 2019 were collected. Patients receiving nutritional therapy within 3 d of ICU admission were divided into EN or PN (including PN+EN) groups. Cox analysis and regression were used to determine relevant factors and establish a nomogram for predicting survival. The 28-d survival rate was significantly better in the EN group compared with the PN/PN+EN group. Risk factors included age, peripheral capillary oxygen saturation, red cell distribution width, international normalised ratio, potassium level, mean corpuscular Hg, myocardial infarction, liver disease, cancer status and nutritional status. The nomogram showed good predictive performance. In ICU patients receiving vasopressor drugs, patients receiving EN had a better survival rate than PN. Our nomogram had favourable predictive value for 28-d survival in patients. However, it needs further validation in prospective trials.
We report our experience with an emergency room (ER) shutdown related to an accidental exposure to a patient with coronavirus disease 2019 (COVID-19) who had not been isolated.
Setting:
A 635-bed, tertiary-care hospital in Daegu, South Korea.
Methods:
To prevent nosocomial transmission of the disease, we subsequently isolated patients with suspected symptoms, relevant radiographic findings, or epidemiology. Severe acute respiratory coronavirus 2 (SARS-CoV-2) reverse-transcriptase polymerase chain reaction assays (RT-PCR) were performed for most patients requiring hospitalization. A universal mask policy and comprehensive use of personal protective equipment (PPE) were implemented. We analyzed effects of these interventions.
Results:
From the pre-shutdown period (February 10–25, 2020) to the post-shutdown period (February 28 to March 16, 2020), the mean hourly turnaround time decreased from 23:31 ±6:43 hours to 9:27 ±3:41 hours (P < .001). As a result, the proportion of the patients tested increased from 5.8% (N=1,037) to 64.6% (N=690) (P < .001) and the average number of tests per day increased from 3.8±4.3 to 24.7±5.0 (P < .001). All 23 patients with COVID-19 in the post-shutdown period were isolated in the ER without any problematic accidental exposure or nosocomial transmission. After the shutdown, several metrics increased. The median duration of stay in the ER among hospitalized patients increased from 4:30 hours (interquartile range [IQR], 2:17–9:48) to 14:33 hours (IQR, 6:55–24:50) (P < .001). Rates of intensive care unit admissions increased from 1.4% to 2.9% (P = .023), and mortality increased from 0.9% to 3.0% (P = .001).
Conclusions:
Problematic accidental exposure and nosocomial transmission of COVID-19 can be successfully prevented through active isolation and surveillance policies and comprehensive PPE use despite longer ER stays and the presence of more severely ill patients during a severe COVID-19 outbreak.
The present study investigated the association between dietary patterns and hypertension applying the Chinese Dietary Balance Index-07 (DBI-07).
Design:
A cross-sectional study on adult nutrition and chronic disease in Inner Mongolia. Dietary data were collected using 24 h recall over three consecutive days and weighing method. Dietary patterns were identified using principal components analysis. Generalized linear models and multivariate logistic regression models were used to examine the associations between DBI-07 and dietary patterns, and between dietary patterns and hypertension.
Setting:
Inner Mongolia (n 1861).
Participants:
A representative sample of adults aged ≥18 years in Inner Mongolia.
Results:
Four major dietary patterns were identified: ‘high protein’, ‘traditional northern’, ‘modern’ and ‘condiments’. Generalized linear models showed higher factor scores in the ‘high protein’ pattern were associated with lower DBI-07 (βLBS = −1·993, βHBS = −0·206, βDQD = −2·199; all P < 0·001); the opposite in the ‘condiments’ pattern (βLBS = 0·967, βHBS = 0·751, βDQD = 1·718; all P < 0·001). OR for hypertension in the highest quartile of the ‘high protein’ pattern compared with the lowest was 0·374 (95 % CI 0·244, 0·573; Ptrend < 0·001) in males. OR for hypertension in the ‘condiments’ pattern was 1·663 (95 % CI 1·113, 2·483; Ptrend < 0·001) in males, 1·788 (95 % CI 1·155, 2·766; Ptrend < 0·001) in females.
Conclusions:
Our findings suggested a higher-quality dietary pattern evaluated by DBI-07 was related to decreased risk for hypertension, whereas a lower-quality dietary pattern was related to increased risk for hypertension in Inner Mongolia.
Oxygen vacancies have a significant impact on the structure and electrical/magnetic properties of doped manganites. Magnetic La0.5Sr0.5MnO3 (LSMO) films were epitaxial grown on (001) SrTiO3 substrate by pulsed laser deposition. Structural studies from the x-ray diffraction suggest that the as-grown films are fully constrained by the substrate with the thickness ranging from 8 to 40 nm. By examining the valence of Mn by x-ray photoelectron spectroscopy and x-ray absorption spectroscopy, we find the ratio of Mn4+/Mn3+ increases along with the increased film thickness, which implies that the oxygen vacancies concentration induced by tensile strain correspondingly decreases. Therefore, the magnetization of LSMO is depressed with the exchange bias effect arising, and the electrical conductivity decreases significantly. This work builds a bridge between modulation of electric/magnetic properties and epitaxial strain in LSMO films.
Very high cycle bending fatigue behaviors of FV520B steel under fretting wear were studied by the ultrasonic fatigue technique. The specimen system for ultrasonic bending testing was designed and the stress distribution of fatigue specimen was obtained by finite element method. The microstructure of FV520B steel was characterized by means of optical microscope, transmission electron microscope, and energy-dispersive spectroscope. The P–S–N curve was drawn based on fatigue data. The micromorphology characteristics of fretting wear surface and fracture surface for fatigue specimen were observed. The results indicate that the microstructure of FV520B steel is mainly composed of lath martensite, ferrite, and precipitation particles, with some randomly distributed internal inclusions. The P–S–N curve shows that there exists no “conventional fatigue limit” and the fatigue life decreases continuously with the increase of applied stress Smax. Most of fatigue cracks are observed on fractography and initiate from the overlap region of fretting wear zone and stress concentration zone. The fracture failure for tested specimen is ascribed to fretting wear and bending vibration fatigue.
Hepatitis C virus (HCV) has become a global public health problem. Many studies have been conducted to identify risk factors for HCV infection. However, some of these studies reported inconsistent results. Using data collected from 11 methadone clinics, we fit both a non-spatial logistical regression and a geographically weighted logistic regression to analyse the association between HCV infection and some factors at the individual level. This study enrolled 5401 patients with 30·0% HCV infection prevalence. The non-spatial logistical regression found that injection history, drug rehabilitation history and senior high-school education or above were related to HCV infection; and being married was negatively associated with HCV infection. Using the spatial model, we found that Yi ethnicity was negatively related to HCV infection in 62·0% of townships, and being married was negatively associated with HCV infection in 81·0% of townships. Senior high-school education or above was positively associated with HCV infection in 55·2% of townships of the Yi Autonomous Prefecture. The spatial model offers better understanding of the geographical variations of the risk factors associated with HCV infection. The geographical variations may be useful for customizing intervention strategies for local regions for more efficient allocation of limited resources to control transmission of HCV.
Meningiomas account for approximately 24-30% of primary intracranial neoplasms. Histopathologic grade and degree of resection are two major prognostic factors. The aim of this study was to determine the factors associated with the prognosis of meningioma.
Methods:
We used immunohistochemistry to analyze the expression levels of Vav3, SPARC, p-Akt, cyclin D1, and Ki-67 in 287 meningiomas of all grades.
Results:
The expression of Vav3, SPARC, p-Akt, cyclin D1, and Ki- 67 significantly increased with meningioma grade (p<0.01), and was higher in brain-invasive meningiomas compared to non-invasive meningiomas (WHO grade I) (p<0.05). Furthermore, the expression of Vav3, p-Akt, and Ki-67 was higher in recurrent meningiomas compared to non-recurrent meningiomas (WHO grade I) (p<0.05).
Conclusion:
The expression of Vav3, SPARC, p-Akt, cyclin D1, and Ki-67 in meningiomas appears to correlate with meningioma invasiveness, aggressiveness, and recurrence.
By using the 1-m reflecting telescope at Weihai Observatory of Shandong University, the transit observations of seven stars are carried out to accurately estimate the physical parameters of extrasolar planets. Besides, a new high-resolution spectrograph (WES) was installed on this telescope for radial velocity measurements on exoplanets, we will show some preliminary results.
We study some convergence of two kinds of implicit iteration processes for approximating common fixed points of a pseudo-contractive semigroup in uniformly convex Banach spaces with uniformly Gateaux differential norms. As special cases, we get some convergence of the implicit iteration processes for approximating common fixed points of a nonexpansive semigroup in uniformly smooth Banach spaces and give a positive answer to an open problem proposed by Xu in Bull. Austral. Math. Soc. (2005). The results presented in this paper generalise some corresponding results from Osilike in Panamer. Math. J. (2004), Suzuki in Proc. Amer. Math. Soc. (2002) and Xu in Bull. Austral. Math. Soc. (2005).
