Epistaxis is the most common ENT emergency. This study aimed to assess one-year mortality rates in patients admitted to a large teaching hospital.
This study was a retrospective case note analysis of all patients admitted to the Queen Elizabeth University Hospital in Glasgow with epistaxis over a 12-month period.
The one-year overall mortality for a patient admitted with epistaxis was 9.8 per cent. The patients who died were older (mean age 77.2 vs 68.8 years; p = 0.002), had a higher Cumulative Illness Rating Scale-Geriatric score (9.9 vs 6.7; p < 0.001) and had a higher performance status score (2 or higher vs less than 2; p < 0.001). Other risk factors were a low admission haemoglobin level (less than 128 g/dl vs 128 g/dl or higher; p = 0.025), abnormal coagulation (p = 0.004), low albumin (less than 36 g/l vs more than 36 g/l; p < 0.001) and longer length of stay (p = 0.046).
There are a number of risk factors associated with increased mortality after admission with epistaxis. This information could help with risk stratification of patients at admission and enable the appropriate patient support to be arranged.
As demonstrated by neuroimaging data, the human brain contains systems that control responses to threat. The revised Reinforcement Sensitivity Theory of personality predicts that individual differences in the reactivity of these brain systems produce anxiety and fear-related personality traits. Here we discuss some of the challenges in testing this theory and, as an example, present a pilot study that aimed to dissociate brain activity during pursuit by threat and goal conflict. We did this by translating the Mouse Defense Test Battery for human fMRI use. In this version, dubbed the Joystick Operated Runway Task (JORT), we repeatedly exposed 24 participants to pursuit and goal conflict, with and without threat of electric shock. The runway design of JORT allowed the effect of threat distance on brain activation to be evaluated independently of context. Goal conflict plus threat of electric shock caused deactivation in a network of brain areas that included the fusiform and middle temporal gyri, as well as the default mode network core, including medial frontal regions, precuneus and posterior cingulate gyrus, and laterally the inferior parietal and angular gyri. Consistent with earlier research, we also found that imminent threat activated the midbrain and that this effect was significantly stronger during the simple pursuit condition than during goal conflict. Also consistent with earlier research, we found significantly greater hippocampal activation during goal conflict than pursuit by imminent threat. In conclusion, our results contribute knowledge to theories linking anxiety disorders to altered functioning in defensive brain systems and also highlight challenges in this research domain.
The catchments of Pine Island Glacier and Thwaites Glacier in the Amundsen Sea Embayment are two of the largest, most rapidly changing, and potentially unstable sectors of the West Antarctic Ice Sheet. They are also neighboring outlets, separated by the topographically unconfined eastern shear margin of Thwaites Glacier and the southwest tributary of Pine Island Glacier. This tributary begins just downstream of the eastern shear margin and flows into the Pine Island ice shelf. As a result, it is a potential locus of interaction between the two glaciers and could result in cross-catchment feedback during the retreat of either. Here, we analyze relative basal reflectivity profiles from three radar sounding survey lines collected using the UTIG HiCARS radar system in 2004 and CReSIS MCoRDS radar system in 2012 and 2014 to investigate the extent and character of ocean access beneath the southwest tributary. These profiles provide evidence of ocean access ~12 km inland of the 1992–2011 InSAR-derived grounding line by 2014, suggesting either retreat since 2011 or the intrusion of ocean water kilometers inland of the grounding line.
Depression and obesity are highly prevalent, and major impacts on public health frequently co-occur. Recently, we reported that having depression moderates the effect of the FTO gene, suggesting its implication in the association between depression and obesity.
To confirm these findings by investigating the FTO polymorphism rs9939609 in new cohorts, and subsequently in a meta-analysis.
The sample consists of 6902 individuals with depression and 6799 controls from three replication cohorts and two original discovery cohorts. Linear regression models were performed to test for association between rs9939609 and body mass index (BMI), and for the interaction between rs9939609 and depression status for an effect on BMI. Fixed and random effects meta-analyses were performed using METASOFT.
In the replication cohorts, we observed a significant interaction between FTO, BMI and depression with fixed effects meta-analysis (β=0.12, P = 2.7 × 10−4) and with the Han/Eskin random effects method (P = 1.4 × 10−7) but not with traditional random effects (β = 0.1, P = 0.35). When combined with the discovery cohorts, random effects meta-analysis also supports the interaction (β = 0.12, P = 0.027) being highly significant based on the Han/Eskin model (P = 6.9 × 10−8). On average, carriers of the risk allele who have depression have a 2.2% higher BMI for each risk allele, over and above the main effect of FTO.
This meta-analysis provides additional support for a significant interaction between FTO, depression and BMI, indicating that depression increases the effect of FTO on BMI. The findings provide a useful starting point in understanding the biological mechanism involved in the association between obesity and depression.
The Dawn spacecraft orbited Asteroid (4) Vesta for a year, and returned disk-resolved images and spectra covering visible and near-infrared wavelengths at scales as high as 20 m/pix. The visible geometric albedo of Vesta is ~ 0.36. The disk-integrated phase function of Vesta in the visible wavelengths derived from Dawn approach data, previous ground-based observations, and Rosetta OSIRIS observations is consistent with an IAU H-G phase law with H=3.2 mag and G=0.28. Hapke's modeling yields a disk-averaged single-scattering albedo of 0.50, an asymmetry factor of -0.25, and a roughness parameter of ~20 deg at 700 nm wavelength. Vesta's surface displays the largest albedo variations observed so far on asteroids, ranging from ~0.10 to ~0.76 in geometric albedo in the visible wavelengths. The phase function of Vesta displays obvious systematic variations with respect to wavelength, with steeper slopes within the 1- and 2-micron pyroxene bands, consistent with previous ground-based observations and laboratory measurement of HED meteorites showing deeper bands at higher phase angles. The relatively high albedo of Vesta suggests significant contribution of multiple scattering. The non-linear effect of multiple scattering and the possible systematic variations of phase function with albedo across the surface of Vesta may invalidate the traditional algorithm of applying photometric correction on airless planetary surfaces.
We draw a distinction between defensive and predatory forms of aggression, and how these forms relate to basic neuropsychological systems, especially the Fight-Flight-Freeze-System (FFFS; putatively related to defensive aggression), and the Behavioural Approach System (BAS; putatively related to predatory aggression). These systems may help further to account for proximal brain processes and personality influences in the context of sex differences.
The effects of intercropping wheat with faba bean (Denmark, Germany, Italy and UK) and wheat with pea (France), in additive and replacement designs on grain nitrogen and sulphur concentrations were studied in field experiments in the 2002/03, 2003/04 and 2004/05 growing seasons. Intercropping wheat with grain legumes regularly increased the nitrogen concentration of the cereal grain, irrespective of design or location. Sulphur concentration of the cereal was also increased by intercropping, but less regularly and to a lesser extent compared with effects on nitrogen concentration. Nitrogen concentration (g/kg) in wheat additively intercropped with faba bean was increased by 8% across all sites (weighted for inverse of variance), but sulphur concentration was only increased by 4%, so N:S ratio was also increased by 4%. Intercropping wheat with grain legumes increased sodium dodecyl sulphate (SDS)-sedimentation volume. The effect of intercropping on wheat nitrogen concentration was greatest when intercropping had the most deleterious effect on wheat yield and the least deleterious effect on pulse yield. Over all sites and seasons, and irrespective of whether the design was additive or replacement, increases in crude protein concentration in the wheat of 10 g/kg by intercropping with faba bean were associated with 25–30% yield reduction of the wheat, compared with sole-cropped wheat. It was concluded that the increase in protein concentration of wheat grain in intercrops could be of economic benefit when selling wheat for breadmaking, but only if the bean crop was also marketed effectively.
This paper reviews the successful N'Dama cattle genetic improvement programme implemented in a low input production system at the International Trypanotolerance Centre (ITC) in 1994, in The Gambia. The first part of the paper presents the genetic improvement programme. The second part deals with the analysis of the genetic improvement programme. The success of the genetic improvement programme expressed through genetic progress and the benefits for the farmers is encouraging. Recommendations to strengthen the implementation process in the field are made.
Airborne radio-echo sounding investigations in the upper reaches of Bailey Ice Stream and Slessor Glacier, Coats Land, East Antarctica, have shown that enhanced-flow tributaries are associated with well-defined areas of relatively thicker ice, and are separated from each other by areas of relatively thinner ice. A numerical modelling study has revealed that while internal ice deformation might account for all the observed flow in inter-tributary areas and the majority in the Slessor tributaries, a significant proportion of the flow of Bailey tributary is attributable to basal motion. Further, investigations of depth-corrected basal reflection power indicate that the bed underlying both Bailey and Slessor enhanced-flow tributaries is significantly smoother than in the slower-moving inter-tributary areas. It is thus proposed that enhanced motion within Bailey tributary (and also perhaps Slessor) may be facilitated by a reduction in basal roughness, caused by the accumulation of water and/or sediments within subglacial valleys, or by the erosion and smoothing of bed obstacles.
The polarization behaviour of radar waves transmitted through two Antarctic ice shelves has been investigated using a step frequency radar with a centre frequency of 300 MHz and a bandwidth of 150 MHz. One site was on Brunt Ice Shelf at a site near Halley station, and 17 sites were on George VI Ice Shelf near the southern ice front. Birefringence in the ice dominated the behaviour on Brunt Ice Shelf, where the anisotropy in the effective permittivity was found to be about 0.14%. On George VI Ice Shelf, a highly anisotropic reflecting surface was the controlling feature, suggesting a fluted ice-shelf base formed by oceanographic currents.
We present newly acquired airborne radar data showing ice thickness and surface elevation for Pine Island Glacier, Antarctica. These data, when combined with earlier measurements, suggest the presence of a lightly grounded area immediately above the grounding line of Pine Island Glacier. We identify this region as an “ice plain”. It lies close to the centre line of the glacier, has an elevation above buoyancy of <50 m and extends inland for >28 km. The upstream edge of the ice plain is defined by a “coupling line”. The configuration of the ice plain implies that nearby thinning of the ice stream would result in substantial grounding-line retreat. We suggest that the grounding-line retreat of Pine Island Glacier, observed between 1992 and 1996, probably commenced sometime after 1981.
Successful growth of (001 )ErP0.6As0.4 single crystal film on (001) GaAs has been demonstrated. The epitaxial metallic layers reproducibly showed lattice mismatch below 5 10-4. This is, to our knowledge, the first report of a stable, epitaxial and lattice-matched metal/compound semiconductor heterostructure. The ErP0.6As0.4/n-GaAs diodes yielded excellent I-V characteristics with an ideality factor of 1.1 and barrier height of 0.88 eV. For a 240 A-thick film, metallic behavior was observed with resistivities of 25 and 86 µΩcm at 1.5 K and room temperature, respectively. As the other Er compounds ErP, ErAs, ErSb and ErSi2, ErP0.6As0.4 presents an abrupt drop in resistivity in the vicinity of the liquid helium temperature, due to a paramagnetic to antiferromagnetic phase transition.
