In patients with β-lactam allergies, administration of non–β-lactam surgical prophylaxis is associated with increased risk of infection. Although many patients self-report β-lactam allergies, most are unconfirmed or mislabeled. A quality improvement process, utilizing a structured β-lactam allergy tool, was implemented to improve the utilization of preferred β-lactam surgical prophylaxis.
In product design engineering (PDE), ideation involves the generation of technical behaviours and physical structures to address specific functional requirements. This differs from generic creative ideation tasks, which emphasise functional and technical considerations less. To advance knowledge about the neural basis of PDE ideation, we present the first fMRI study on professional product design engineers practising in industry. We aimed to explore brain activation during ideation, and compare activation in open-ended and constrained tasks. Imagery manipulation tasks were contrasted with ideation tasks in a sample of 29 PDE professionals. The key findings were: (1) PDE ideation is associated with greater activity in left cingulate gyrus; (2) there were no significant differences between open-ended and constrained tasks; and (3) a preliminary association with activity in the right superior temporal gyrus was also observed. The results are consistent with existing fMRI work on generic creative ideation, suggesting that PDE ideation may share a number of similarities at the neural level. Future work includes: functional connectivity analysis of open-ended and constrained ideation to further investigate potential differences; investigating the effects of aspects of design expertise/training on processing; and the use of novelty measures directly linked to the designer’s internal processing in fMRI analysis.
Common practices for invasive species control and management include physical, chemical, and biological approaches. The first two approaches have clear limitations and may lead to unintended (negative) consequences, unless carefully planned and implemented. For example, physical removal rarely completely eradicates the targeted invasive species and can cause disturbances that facilitate new invasions by nonnative species from nearby habitats. Chemical treatments can harm native, and especially rare, species through unanticipated side effects. Biological methods may be classified as biocontrol and the ecological approach. Similar to physical and chemical methods, biocontrol also has limitations and sometimes leads to unintended consequences. Therefore, a relatively safer and more practical choice may be the ecological approach, which has two major components: (1) restoration of native species and (2) biomass manipulation of the restored community, such as selective grazing or prescribed burning (to achieve and maintain viable population sizes). Restoration requires well-planned and implemented planting designs that consider alpha-, beta-, and gamma-diversity and the abundance of native and invasive component species at local, landscape, and regional levels. Given the extensive destruction or degradation of natural habitats around the world, restoration could be most effective for enhancing ecosystem resilience and resistance to biotic invasions. At the same time, ecosystems in human-dominated landscapes, especially those newly restored, require close monitoring and careful intervention (e.g., through biomass manipulation), especially when successional trajectories are not moving as intended. Biomass management frequently uses prescribed burning, grazing, harvesting, and thinning to maintain overall ecosystem health and sustainability. Thus, the resulting optimal, balanced, and relatively stable ecological conditions could more effectively limit the spread and establishment of invasive species. Here we review the literature (especially within the last decade) on ecological approaches that involve biodiversity, biomass, and productivity, three key community/ecosystem variables that reciprocally influence one another. We focus on the common and most feasible ecological practices that can aid in resisting new invasions and/or suppressing the dominance of existing invasive species. We contend that, because of the strong influences from neighboring areas (i.e., as exotic species pools), local restoration and management efforts in the future need to consider the regional context and projected climate changes.
A staple theme in clinical psychology, emotion regulation, or the ability to manage one's emotions, is directly linked with personal wellbeing and the ability to effectively navigate the social world. Until recently, this concept has been limited to a focus on intrapersonal processes, such as suppression. Less emphasis has been placed on developmental, social, and cultural aspects of emotion regulation. We argue here that as social beings, our engagement in emotion regulation may often occur interpersonally, with trusted others helping us to regulate our emotions. This review will highlight recent research on interpersonal emotion regulation processes.
A major goal of Gardner, Ryan, and Snoeyink (2018) was to determine what steps are needed moving forward in examining gender representation in industrial and organizational (I-O) psychology. Specifically, on the topic of pay differences, we highlight that gender differences in pay are in part due to differences in negotiation behaviors and/or experiences. Prior research demonstrates that female negotiators receive greater backlash than male negotiators—a possible explanation to why men tend to negotiate more often and more successfully than women (Bowles, Babcock, & Lai, 2007). Based on this evidence, one next step in moving forward should involve providing resources and knowledge to improve negotiation skills and practices specifically aimed at eliminating differences between women and men in both propensity to negotiate and the evaluation/consequences of negotiating.
Impaired β-cell development and insulin secretion are characteristic of intrauterine growth-restricted (IUGR) fetuses. In normally grown late gestation fetal sheep pancreatic β-cell numbers and insulin secretion are increased by 7–10 days of pulsatile hyperglycemia (PHG). Our objective was to determine if IUGR fetal sheep β-cell numbers and insulin secretion could also be increased by PHG or if IUGR fetal β-cells do not have the capacity to respond to PHG. Following chronic placental insufficiency producing IUGR in twin gestation pregnancies (n=7), fetuses were administered a PHG infusion, consisting of 60 min, high rate, pulsed infusions of dextrose three times a day with an additional continuous, low-rate infusion of dextrose to prevent a decrease in glucose concentrations between the pulses or a control saline infusion. PHG fetuses were compared with their twin IUGR fetus, which received a saline infusion for 7 days. The pulsed glucose infusion increased fetal arterial glucose concentrations an average of 83% during the infusion. Following the 7-day infusion, a square-wave fetal hyperglycemic clamp was performed in both groups to measure insulin secretion. The rate of increase in fetal insulin concentrations during the first 20 min of a square-wave hyperglycemic clamp was 44% faster in the PHG fetuses compared with saline fetuses (P<0.05). There were no differences in islet size, the insulin+ area of the pancreas and of the islets, and β-cell mass between groups (P>0.23). Chronic PHG increases early phase insulin secretion in response to acute hyperglycemia, indicating that IUGR fetal β-cells are functionally responsive to chronic PHG.
Early life exposures affect health and disease across the life course and potentially across multiple generations. The Clinical and Translational Research Institutes (CTSIs) offer an opportunity to utilize and link existing databases to conduct lifespan research.
A survey with Lifespan Domain Taskforce expert input was created and distributed to lead lifespan researchers at each of the 64 CTSIs. The survey requested information regarding institutional databases related to early life exposure, child-maternal health, or lifespan research.
Of 64 CTSI, 88% provided information on a total of 130 databases. Approximately 59% (n=76/130) had an associated biorepository. Longitudinal data were available for 72% (n=93/130) of reported databases. Many of the biorepositories (n=44/76; 68%) have standard operating procedures that can be shared with other researchers.
The majority of CTSI databases and biorepositories focusing on child-maternal health and lifespan research could be leveraged for lifespan research, increased generalizability and enhanced multi-institutional research in the United States.
We have detected maser emission from the 36.2 GHz (4−1 → 30E) methanol transition towards NGC 4945. This emission has been observed in two separate epochs and is approximately five orders of magnitude more luminous than typical emission from this transition within our Galaxy. NGC 4945 is only the fourth extragalactic source observed hosting class I methanol maser emission. Extragalactic class I methanol masers do not appear to be simply highly-luminous variants of their galactic counterparts and instead appear to trace large-scale regions where low-velocity shocks are present in molecular gas.
We describe the performance of the Boolardy Engineering Test Array, the prototype for the Australian Square Kilometre Array Pathfinder telescope. Boolardy Engineering Test Array is the first aperture synthesis radio telescope to use phased array feed technology, giving it the ability to electronically form up to nine dual-polarisation beams. We report the methods developed for forming and measuring the beams, and the adaptations that have been made to the traditional calibration and imaging procedures in order to allow BETA to function as a multi-beam aperture synthesis telescope. We describe the commissioning of the instrument and present details of Boolardy Engineering Test Array’s performance: sensitivity, beam characteristics, polarimetric properties, and image quality. We summarise the astronomical science that it has produced and draw lessons from operating Boolardy Engineering Test Array that will be relevant to the commissioning and operation of the final Australian Square Kilometre Array Path telescope.
The aims of the present study were to provide nationally representative data on fruit and vegetable consumption in Vietnam, and to assess the accuracy of the reported numbers of ‘standard servings’ consumed. Data analysed were from a multi-stage stratified cluster survey of 14 706 participants (46·5 % males, response proportion 64·1 %) aged 25−64 years in Vietnam. Measurements were made in accordance with the WHO STEPwise approach to surveillance of non-communicable diseases (STEPS) protocols. Approximately 80 % of Vietnamese people reported having less than five servings of fruit and vegetables daily in a typical week. Fruit and vegetable intake reported in ‘standard serving’ sizes was positively correlated with levels of education completed and household income (P<0·001 for trend). The correlations between summary values for each province reflect some known demographic, geographical and climatic characteristics of the country. For example, provinces at higher latitude had higher mean servings of vegetables (r 0·90), and provinces with higher proportions of urban population had higher mean servings of fruit (r 0·40). In conclusion, about eight in ten Vietnamese people aged 25–64 years did not meet WHO recommendations for daily consumption of at least five servings of fruit and vegetables. On the basis of the consistency of the data collected with other estimates and with physical and demographic characteristics of the country, the WHO STEPS instrument has construct validity for measuring fruit and vegetable intake, but with two issues identified. The issues were seasonal variation in reporting and a limitation on the usefulness of the information for associative analyses.
Despite high levels of mental illness, Vietnamese youth have limited access to mental health care. Internet interventions, evidence-based psychotherapy treatments delivered through the internet, have the potential to increase access to mental health for youth in Vietnam. This study explored the perceptions of youths and parents toward internet interventions for youth mental health.
Four focus groups were conducted with youths (n = 20) and parents (n = 20) in Danang, Vietnam. The Technology Acceptance Model was used a framework for focus group questions. The data were analyzed using direct content analysis.
Most youths and parents agreed that the internet serves well as a care delivery model. Participants expressed that the web would be useful for psychoeducation and sharing and receiving information with others. Both groups reported lack of awareness of web-based interventions and logistical concerns regarding access as main barriers. In addition, many parents were concerned about internet addiction. Specific adaptations in Vietnam such as standalone internet service centers and partnering with local organizations may benefit uptake of internet interventions.
This study suggests that internet-based programs for youth mental health, particularly interventions incorporating psychoeducation and social networking components, will be well received in Vietnam. Barriers need to be addressed to successfully implement internet-based treatment. Future initiatives should incorporate acceptance models to improve development of internet interventions for youth.
This paper describes the system architecture of a newly constructed radio telescope – the Boolardy engineering test array, which is a prototype of the Australian square kilometre array pathfinder telescope. Phased array feed technology is used to form multiple simultaneous beams per antenna, providing astronomers with unprecedented survey speed. The test array described here is a six-antenna interferometer, fitted with prototype signal processing hardware capable of forming at least nine dual-polarisation beams simultaneously, allowing several square degrees to be imaged in a single pointed observation. The main purpose of the test array is to develop beamforming and wide-field calibration methods for use with the full telescope, but it will also be capable of limited early science demonstrations.
We propose a Bayesian method to measure the total Galactic extinction parameters, RV and AV. Validation tests based on the simulated data indicate that the method can achieve the accuracy of around 0.01 mag. We apply this method to the SDSS BHB stars in the northern Galactic cap and find that the derived extinctions are highly consistent with those from Schlegel et al. (1998). It suggests that the Bayesian method is promising for the extinction estimation, even the reddening values are close to the observational errors.
We match the XMM-Newton 3XMMi-DR4 catalog with the Sloan Digital Sky Survey (SDSS) Data Release 10 and the United Kingdom Infrared Deep Sky Survey (UKIDSS) Data Release 9. Based on this X-ray/optical/infrared catalog, we probe the distribution of various types of X-ray emitters in the multidimensional parameter space. It is found that quasars, galaxies and stars have some kind distribution rule, especially for stars. The result shows that only using the X-ray/optical features, stars are difficult to discriminate from galaxies and quasars, the added information from infrared band is very helpful to improve the classification result of any classifier. Comparing the classification accuracy of random forests with that of rotation forests, rotation forests show better performance.
Zenker's diverticulum is a propulsion diverticulum in the pharynx. Current practice for the management of symptomatic pharyngeal pouches includes endoscopic pharyngeal stapling, performed trans-orally, and external approaches via a cervical incision. There is no published recommendation on how to approach diverticula with extension into the mediastinum, which may not be adequately treated with the above methods.
We describe two cases in which thoracoscopic mobilisation of Zenker's diverticulum was performed using video-assisted thoracoscopic surgery together with traditional transcervical mobilisation and excision of the pouch. This allowed safe surgical access to the inferior limit of the pouch, and delivery of the sac into the neck incision following division of any inferior adhesions (to the great vessels in one case).
In the first report of this technique, we describe a thorough, safe method of dissecting large diverticula that extend into the mediastinum, which minimises the risk to mediastinal structures.
A rapid nano-indentation measurement technique is employed to produce surface maps of hardness. Each indentation cycle requires less than three seconds, including surface approach, contact detection, force application, withdrawal, and movement to the next indentation site. Traditional nano-indentation analyses are applied to the force-displacement measurements from each indentation, but information storage and presentation owe much to scanning-probe technology. Consequently, this nano-indentation technique produces three-dimensional images of mechanical properties which are stored and manipulated just like scanned images.
In the present work, we use this technique to map the hardness of a SAC 305 solder joint with gold plating. After extended isothermal aging, the solder joint comprises three constituents: a tin-rich matrix, a bulk intermetallic AuSn4, and an interfacial intermetallic (Cu, Ni, Au)6Sn5. The softest material is the tin-rich matrix, which has a hardness of 0.51±0.07 GPa. The hardness of the bulk intermetallic is 2.12±0.18 GPa. The interfacial intermetallic has extraordinary hardness— greater than 8 GPa. Under uniform plastic strain, the mismatch in hardness between the interfacial intermetallic and surrounding material may increase the local stress intensity factor which drives interfacial fracture.
