The National Institute of Neurological Disease and Stroke-Canadian Stroke Network (NINDS-CSN) 5-minute neuropsychology protocol consists of only verbal tasks, and is proposed as a brief screening method for vascular cognitive impairment. We evaluated its feasibility within two weeks after stroke and ability to predict the development of post-stroke dementia (PSD) at 3 months after stroke.
We prospectively enrolled subjects with ischemic stroke within seven days of symptom onset who were consecutively admitted to 12 university hospitals. Neuropsychological assessments using the NINDS-CSN 5-minute and 60-minute neuropsychology protocols were administered within two weeks and at 3 months after stroke onset, respectively. PSD was diagnosed with reference to the American Heart Association/American Stroke Association statement, requiring deficits in at least two cognitive domains.
Of 620 patients, 512 (82.6%) were feasible for the NINDS-CSN 5-minute protocol within two weeks after stroke. The incidence of PSD was 16.2% in 308 subjects who had completed follow-up at 3 months after stroke onset. The total score of the NINDS-CSN 5-minute protocol differed significantly between those with and without PSD (4.0 ± 2.7, 7.4 ± 2.7, respectively; p < 0.01). A cut-off value of 6/7 showed reasonable discriminative power (sensitivity 0.82, specificity 0.67, AUC 0.74). The NINDS-CSN 5-minute protocol score was a significant predictor for PSD (adjusted odds ratio 6.32, 95% CI 2.65–15.05).
The NINDS-CSN 5-minute protocol is feasible to evaluate cognitive functions in patients with acute ischemic stroke. It might be a useful screening method for early identification of high-risk groups for PSD.
Construction of a new science complex in Osong, Cheongwon-gun, Korea, has allowed the investigation of 14 different Paleolithic localities, excavated during 2005–2007. Here, we investigated localities 1 and 12 of the Mansuri Paleolithic site to obtain chronological information using radiocarbon dating. The soil deposition rates varied from 0.09 to 0.15 mm/yr over the period ranging from 33 to 31 kyr BP for locality 1. Locality 12 samples were more recent, <10 ka, and have similar accumulation rates, averaging 0.11 mm/yr. The soil ages of locality 12 were found to be younger than 10 kyr BP. Results for both soil and organic materials at this locality gave much younger ages at shallower depths than the ages expected by the Korean Paleolithic cultural history for this region. Therefore, these more recent deposits may not be associated with the cultural layers and are interpreted to have been hydrologically modified following emplacement. 14C dates of the soil and organic materials at locality 12 confirm that there is evidence for multiple human occupations throughout the last 9 kyr BP.
A series of annual tree-ring measurements has been performed in order to reconstruct the radiocarbon concentration variation in the Korean atmosphere from AD 1650 to 1850. The absolute ages of the samples were determined using dendrochronology. Alpha-cellulose extraction was applied to prepare the tree-ring samples for precise 14C measurement. The 14C concentrations of the tree rings were then plotted with the dendrochronological ages and showed that during the period AD 1650–1850, the discrepancy in 14C concentration in the Korean atmosphere from IntCal data is small enough to use IntCal data without any further correction. This is nearly one third of the average offset of the 400 yr from AD 1250 to 1650. One of the probable causes for the regional offset around Korea is the contribution of 14C-depleted CO2 released from the northern Pacific Ocean, where old deep water upwells to the surface. It is likely that the release rate of 14C-depleted CO2 decreased due to the temperature change during the Little Ice Age.
This study examined changes in health-related quality of life (HRQoL) and quality of care (QoC) as perceived by terminally ill cancer patients and a stratified set of HRQoL or QoC factors that are most likely to influence survival at the end of life (EoL).
We administered questionnaires to 619 consecutive patients immediately after they were diagnosed with terminal cancer by physicians at 11 university hospitals and at the National Cancer Center in Korea. Subjects were followed up over 161.2 person-years until their deaths. We measured HRQoL using the core 30-item European Organization for Research and Treatment of Cancer Quality of Life Questionnaire, and QoC using the Quality Care Questionnaire–End of Life (QCQ–EoL). We evaluated changes in HRQoL and QoC issues during the first three months after enrollment, performing sensitivity analysis by using data generated via four methods (complete case analysis, available case analysis, the last observation carried forward, and multiple imputation).
Emotional and cognitive functioning decreased significantly over time, while dyspnea, constipation, and pain increased significantly. Dignity-conserving care, care by healthcare professionals, family relationships, and QCQ–EoL total score decreased significantly. Global QoL, appetite loss, and Eastern Cooperative Oncology Group Performance Status (ECOG–PS) scores were significantly associated with survival.
Significance of results:
Future standardization of palliative care should be focused on assessment of these deteriorated types of quality. Accurate estimates of the length of life remaining for terminally ill cancer patients by such EoL-enhancing factors as global QoL, appetite loss, and ECOG–PS are needed to help patients experience a dignified and comfortable death.
We present a rapid and sensitive surface acoustic wave (SAW) immunosensor that utilizes gold staining as a signal enhancement method. A sandwich immunoassay was performed on sensing area of the SAW sensor, which could specifically capture and detect cardiac markers (cardiac troponin I (cTnI), creatine kinase (CK)-MB, and myoglobin). The analytes in human serum were captured on gold nanoparticles (AuNPs) that were conjugated in advance with detection antibodies. Introduction of these complexes to the capture antibody-immobilized sensor surface resulted in a classic AuNP-based sandwich immunoassay format that has been used for signal amplification. In order to achieve further signal enhancement, a gold staining method was performed, which demonstrated that it is possible to obtain gold staining-mediated signal augmentation on a mass-sensitive device. The sensor response due to gold staining varied as a function of cardiac marker concentration.
In the present paper, quasilinear development of anisotropy-driven electromagnetic instabilities is computed on the basis of recently formulated empirical wave dispersion relation and compared against exact numerical calculation based upon transcendental plasma dispersion function and exact numerical roots. Upon comparison with the exact method it is demonstrated that the empirical model provides reasonable results. The present findings may be relevant to space physical application, as the present paper provides a useful short-cut research method for self-consistent analysis of temporal development of anisotropy-driven instabilities.
The complex dielectric constants of several π-conjugated materials were measured, and generalized Langevin equation was used to analyze the dielectric behavior in the frequency domain. From the results of the fitting the experimental data using the generalized Langevin equation, we suggest that the charge carriers are electrically screened by the neighboring charges through the structural relaxation, and the carriers are not interact each other. We confirmed that the generalized Langevin equation offers a very good approach to analyze and understand the transport properties of charge carriers in π-conjugated materials.
To improve the IPD reliability of NAND flash memory, plasma oxidation was introduced as the post-treatment process of ONO (Oxide/Nitride/Oxide) IPD. The LP-CVD SiO2 modified by plasma oxidation showed the excellent electrical properties. e.g., low leakage current, high breakdown voltage etc. By the analysis of Tof-SIMS and XRR, we could observe the several changes of physical characteristics such as the reduction of impurities (H, N etc.), the increase of oxide density, and the improvement of oxide surface roughness. We found out the appropriate treatment condition to be able to densify oxide layer without the addition of ONO Equivalent Oxide Thickness (EOT). The LP-CVD SiO2 prepared by plasma oxidation was used for the ONO IPD of 50nm NAND flash device and also compared with the conventional LP-CVD SiO2 in the aspect of the IPD reliability.
Determining valid zygosity is a basic and important requirement in a twin study, because misdiagnosing zygosity leads to biased results. The Healthy Twin Study has collected data from adult like-sex twins and their families since 2005. In the study, a questionnaire to determine zygosity was developed comprising four questions; one concerning the degree of resemblance, and three concerning the degree of confusion by the resemblance. Among 2,761 individuals (624 twin pairs) of twin and their families, 406 pairs of twins (mean age 38.3, 63.5% women) with both questionnaire and genotype information were selected to examine the validity of the zygosity questionnaire using 16 short tandem repeat markers. We first determined individual zygosity including undetermined category, and then decided the zygosity of a twin pair using a decision tree. Sensitivity of questionnaire diagnosis was 98.8% for monozygotic (MZ) and 88.9% for dizygotic (DZ) twins, and positive predictive value was 97.2% for MZ and 95.0% for DZ. When we compared correctly and wrongly diagnosed twin pairs, misdiagnosed DZ twins (nine pairs) showed striking similarity in stature or obesity even exceeding that of true MZ twins. Our finding suggests that a parsimonious questionnaire method of diagnosing the zygosity will be useful, and adding physical or physiological measurements to a questionnaire of zygosity diagnosis will either confound the correct diagnosis or reduce the efficiency of the study compared with using questionnaire alone or with introducing genotyping.
New stable isotope and noble gas data obtained from fumarolic and bubbling gases and hot spring waters sampled from Deception Island, Antarctica, were analysed to constrain the geochemical features of the island's active hydrothermal system and magmatism in the Bransfield back-arc basin. The 3He/4He ratios of the gases (< 9.8 × 10-6), which are slightly lower than typical MORB values, suggest that the Deception Island magma was generated in the mantle wedge of a MORB-type source but the signature was influenced by the addition of radiogenic 4He derived from subducted components in the former Phoenix Plate. The N2/He ratios of fumarolic gas are higher than those of typical mantle-derived gases suggesting that N2 was added during decomposition of sediments in the subducting slab. The δ13C values of -5 to -6‰ for CO2 also indicate degassing from a MORB-type mantle source. The H2/Ar- and SiO2 geothermometers indicate that the temperatures in the hydrothermal system below Deception Island range from ~150°C to ~300°C. The δD and δ18O values measured from fumarolic gas and hot spring waters do not indicate any contribution of magmatic water to the samples. The major ionic components and δD-δ18O-δ34S values indicate that hot spring waters are a mixture of local meteoric water and seawater. Mn and SiO2 in spring waters were enriched relative to seawater reflecting water-rock interaction at depth.
Major depressive disorder (MDD) is closely related to stress reactions and serotonin probably underpins the pathophysiology of MDD. Alterations of the hypothalamic-pituitary-adrenal axis at the gene level have reciprocal consequences on serotonin neurotransmission. Glucocorticoid receptor (GR) polymorphisms affect glucocorticoid sensitivity, which is associated with cortisol feedback effects. Therefore, we hypothesised that GR polymorphisms are associated with the susceptibility to MDD and predict the treatment response.
Ninety-six subjects with a minimum score of 17 on the 21-item Hamilton Depression Scale (HAMD) at baseline were enrolled into the present study. The genotypes of GR (N363S, ER22/23EK, Bcl1, and TthIII1 polymorphisms) were analysed. The HAMD score was again measured after 1, 2, 4 and 8 weeks of antidepressant treatment to detect whether the therapeutic effects differed with the GR genotype.
Our subjects carried no N363S or ER22/23EK genetic polymorphisms and three types of Bcl1 and TthIII1 genetic polymorphisms. The C/C genotype and C allele at Bcl1 polymorphism were more frequent in MDD patients than in normal controls (p < 0.01 and p = 0.01, respectively). The genotype distributions did not differ significantly between responders and non-responders.
These results suggest that GR polymorphism cannot predict the therapeutic response after antidepressant administration. However, GR polymorphism (Bcl1) might play a role in the pathophysiology of MDD. Future studies should check this finding in larger populations with different characteristics.
