Introduction: Within Alberta, 30% of patients presenting to emergency with minor traumatic brain injury (mTBI) will receive a CT scan before being sent home, regardless of whether it was clinically indicated. Choosing Wisely (CW) Canada recommends using validated clinical decision support to determine whether a CT scan is necessary for patients presenting with a mTBI. In order to provide patients with information on the risks and benefits of CT scans in mTBI and to encourage discussions between patients and their doctor, the Emergency StrategicClinical Network (ESCN) designed a patient focused information visualization on CT scans for head injuries. Methods: The ESCN, Physician Learning Program and CW Alberta partnered with the Mount Royal University Department of Information Design to develop a patient information visualization (infographic) intervention. Students spent a semester developing these infographics on Choosing Wisely recommendations, which were then presented to stakeholders. A student was then selected to develop a final design. Refinement of the design took place in consultation with clinical experts and tested in two patient focus groups. The final design was evaluated against the International Patient Decision Aid Standards checklist. The infographic was posted in 2 local emergency department waiting rooms. A survey was administered to any patients in the waiting room when volunteers were available. The survey was designed to evaluate whether the tool influenced patient beliefs about the risks and benefits of CT scans, and their willingness to engage in a discussion with their doctor. Results: In a 26 day period, 90 patients consented and completed the survey. Before reading the infographic, 33% of patients thought that after a head injury a CT was always a good idea and 63% thought it was sometimes a good idea. 82% and 91% of patients stated the poster helped them understand the indications and risks of CT imaging for mTBI. After viewing the poster, only 15% of patients felt that a CT was always a good idea after a mTBI. Conclusion: The mTBI patient infographic significantly changed patient perceptions regarding the need for CT scans in the setting of mTBI. This study demonstrates that targeted patient education materials can help support CW recommendations.

When one takes into account the presence of virtual charged states in the quantum vacuum, a nonlinear self-interaction can arise in the propagation of electromagnetic fields. This self-interaction is often referred to as ‘real photon–photon scattering’. When the centre-of-mass energy of colliding photons is much lower than the rest energy of an electron–positron pair, this quantum effect can be included in the classical field equations of motion as a vacuum current and charge density using the Heisenberg–Euler Lagrangian. Using analytical and numerical methods for subcritical fields, the intrinsic solution to Maxwell’s equations has been found for counterpropagating probe and pump plane waves in the presence of vacuum four- and six-wave mixing. In the corresponding all-order solution for the scattered probe, a route to vacuum high-harmonic generation is identified in which a long phase length can compensate for the weakness of interacting fields. The resulting shocks in the probe carrier wave and envelope are studied for different parameter regimes and polarisation set-ups. In this special issue, we study two additional set-ups: that of a slowly varying single-cycle background to highlight the effect of an oscillating background on the probe harmonic spectrum, and that of a few-cycle probe to highlight the smoothing of the harmonic peaks produced by a wider spectrum of probe photons. We also correct sign errors in an earlier publication.

Background: Availability of real-time electronic healthcare data provides new opportunities for rapid-cycle evaluation (RCE) of health technologies, including healthcare delivery and payment programs. We aim to align decision-making processes with stages of RCE to optimize the usefulness and impact of rapid results. Rational decisions about program adoption depend on program effect size in relation to externalities, including implementation cost, sustainability, and likelihood of broad adoption.

Methods: Drawing on case studies and experience from drug safety monitoring, we examine how decision makers have used scientific evidence on complex interventions in the past. We clarify how RCE alters the nature of policy decisions; develop the RAPID framework for synchronizing decision-maker activities with stages of RCE; and provide guidelines on evidence thresholds for incremental decision-making.

Results: In contrast to traditional evaluations, RCE provides early evidence on effectiveness and facilitates a stepped approach to decision making in expectation of future regularly updated evidence. RCE allows for identification of trends in adjusted effect size. It supports adapting a program in midstream in response to interim findings, or adapting the evaluation strategy to identify true improvements earlier. The 5-step RAPID approach that utilizes the cumulating evidence of program effectiveness over time could increase policy-makers' confidence in expediting decisions.

Conclusions: RCE enables a step-wise approach to HTA decision-making, based on gradually emerging evidence, reducing delays in decision-making processes after traditional one-time evaluations.

“In situ x-ray study of the γ- to α-Al2O3 phase transformation during atmospheric pressure oxidation of NiAl(110)” [J. Mater. Res. 21, 3047 (2006)]

The oxidation in air of NiAl(110) was investigated in the temperature range from 870 °C–1200 °C by in situ x-ray diffraction and transmission electron microscopy. Oxidation at 870 °C and 1 bar oxygen leads to the formation of an epitaxial layer of γ-alumina showing an R30° orientation relationship with respect to the underlying substrate. At oxidation temperatures between 950 °C and 1025 °C, we observed a coexistence of epitaxial γ- and polycrystalline δ-Al2O3. The α-Al2O3 starts to form at 1025 °C and the complete transformation of metastable phases to the stable α-alumina phase takes place at 1100 °C. The fcc-hcp martensitic-like transformation of the initial γ-Al2O3 to epitaxial α-Al2O3 was observed. X-ray diffraction and cross-section transmission electron microscopy proved the existence of a continuous epitaxial α-Al2O3 layer between the substrate and the polycrystalline oxide scale, having a thickness of about 150 nm. The relative orientation relationship between the epitaxial alumina and the underlying substrate was found to be NiAl(110) || α-Al2O3 (0001) and [110] NiAl || [1120].

Ion acceleration by lasers is one of the most important innovations in laser-plasma research in recent years. A mechanism that has gained great attention due to the remarkable properties of the accelerated beam is laser acceleration of protons from the rear surface of solid targets. A striking prediction is that these protons are capable of generating images of micro-structures present on this surface. These images might be useful to measure properties of the accelerated beam. In this article, we address the physics of the generation of images of surface structures imprinted into the target back surface with laser-accelerated protons.

Since their discovery, laser accelerated ion beams have been the subject of great interest. The ion beam peak power and beam emittance is unmatched by any conventionally accelerated ion beam. Due to the unique quality, a wealth of applications has been proposed, and the first experiments confirmed their prospects. Laser ion acceleration is strongly linked to the generation and transport of hot electrons by the interaction of ultra-intense laser light with matter. Comparing ion acceleration experiments at laser systems with different beam parameters and using targets of varying thickness, material and temperature, some insight on the underlying physics can be obtained. The paper will present experimental results obtained at different laser systems, first beam quality measurement on laser accelerated heavy ions, and ion beam source size measurements at different laser parameters. Using structured targets, we compare information obtained from micro patterned ion beams about the accelerating electron sheath, and the influence of magnetic fields on the electron transport inside conducting targets.

BOOMERanG has recently resolved structures on the last scattering surface at redshift ˜ 1100 with high signal to noise ratio. We review the technical advances which made this possible, and we focus on the current results for maps and power spectra, with special attention to the determination of the total mass-energy density in the Universe and of other cosmological parameters.

In the 1998-99 flight, BOOMERanG has produced maps of ∼4% of the sky at high Galactic latitudes, at frequencies of 90, 150, 240 and 410 GHz, with resolution ≳ 10'. The faint structure of the Cosmic Microwave Background at horizon and sub-horizon scales is evident in these maps. These maps compare well to the maps recently obtained at lower frequencies by the WMAP experiment. Here we compare the amplitude and morphology of the structures observed in the two sets of maps. We also outline the polarization sensitive version of BOOMERanG, which was flown early this year to measure the linear polarization of the microwave sky at 150, 240 and 350 GHz.

The Arcminute Cosmology Bolometer Array Receiver (Acbar) is a multifrequency millimeter-wave receiver optimized for observations of the Cosmic Microwave Background (CMB) and the Sunyaev-Zel'dovich (SZ) effect in clusters of galaxies. Acbar was installed on the 2.1 m Viper telescope at the South Pole in January 2001 and the results presented here incorporate data through July 2002. The power spectrum of the CMB at 150 GHz over the range ℓ = 150 — 3000 measured by Acbar is presented along with estimates for the values of the cosmological parameters within the context of ΛCDM models. The inclusion of ΩΛ greatly improves the fit to the power spectrum. Three-frequency images of the SZ decrement/increment are also presented for the galaxy cluster 1E0657–67.

We show how estimates of parameters characterizing inflation-based theories of structure formation localized over the past year when large scale structure (LSS) information from galaxy and cluster surveys was combined with the rapidly developing cosmic microwave background (CMB) data, especially from the recent Boomerang and Maxima balloon experiments. All current CMB data plus a relatively weak prior probability on the Hubble constant, age and LSS points to little mean curvature (Ωtot = 1.08±0.06) and nearly scale invariant initial fluctuations (ns = 1.03±0.08), both predictions of (non-baroque) inflation theory. We emphasize the role that degeneracy among parameters in the Lpk = 212 ± 7 position of the (first acoustic) peak plays in defining the Ωtot range upon marginalization over other variables. Though the CDM density is in the expected range (Ωcdmh2 = 0.17 ± 0.02), the baryon density Ωbh2 = 0.030 ± 0.005 is somewhat above the independent 0.019 ± 0.002 nucleosynthesis estimates. CMB+LSS gives independent evidence for dark energy (ΩΛ = 0.66 ± 0.06) at the same level as from supernova (SN1) observations, with a phenomenological quintessence equation of state limited by SN1+CMB+LSS to wQ < −0.7 cf. the wQ=−1 cosmological constant case.

Low-frequency, relativistic, subcycle solitary waves are found in two-dimensional and three-dimensional particle-in-cell (PIC) numerical simulations, as a result of the interaction of ultrashort, high-intensity laser pulses with plasmas. Moreover, nondrifting, subcycle relativistic electromagnetic solitons have been obtained as solutions of the hydrodynamic equations for an electron–ion warm plasma, by assuming the quasi-neutrality character of the plasma response. In addition, the formation of long-living macroscopic soliton-like structures has been experimentally observed by means of the proton imaging diagnostics. Several common features result from these investigations, as, for example, the quasi-neutral plasma response to the soliton radiation, in the long-term evolution of the system, which leads to the almost complete expulsion of the plasma from the region where the electromagnetic radiation is concentrated, even at subrelativistic field intensity. The results of the theoretical investigations are reviewed with special attention to these similarities.

By analytical modeling and numerical simulation, we show that surface modes in moderately overdense plasmas may be excited parametrically by an intense, ultrashort laser pulse. This process has a feedback effect on fast electron generation and may seed a fast distortion of plasma “moving mirrors.”

Collective absorption, so far determined by numerical simulations, is explained in physical terms for cold and warm plasma. After deducing a few general relations for flat targets, interface phase mixing and nonadiabatic electron acceleration in the skin layer are identified as the main physical processes leading to irreversibility, that is, to absorption.

The effect of surface deformations on laser absorption and transport in overdense plasma targets is investigated with 2D Vlasov simulations. Fractional absorption increases strongly with the deformation depth and scales weakly with density or intensity. Fast electrons generated in the skin layer have high tangential velocity directed toward the deformation center. In the case of a single, smooth deformation, they may be collimated by magnetic fields into a single jet which penetrates deeply into the plasma. Small-scale surface corrugations are able to destroy the collimation of a single jet leading to multiple jets or diffuse electron sprays.

2D, low-frequency, relativistic solitary waves have been found recently in 2D. Particle-in-cell simulations of ultraintense laser pulses propagating in underdense plasmas, in regimes where the laser pulse undergoes energy depletion and downshifting of its frequency. These slowly propagating, spatially localized, electromagnetic structures represent an important channel of energy conversion from the laser pulse to the plasma. Pulse energy can also be converted into fast propagating structures, associated with collimated beams of ultrarelativistic electrons. Lienard–Wiechert magnetic field structures have been observed in PIC simulations to move together with focalized ultrarelativistic electron beams in the plasma.

Measurements of x-ray diffraction (XRD) profiles have been performed on commercially pure Fe and Al powders, cryomilled Fe–3 wt.% Al powders, cold pressed (CP) pure Fe and Al, hot pressed (HP) and hot isostatically pressed (HIP) Fe–3 wt.% Al. Scherrer equation (SE), integral breadth analysis (IBA), and single-line approximation (SLA) methods have been employed to extract grain size and microstrain. The results demonstrate that, in the case of the cryomilled nanocrystalline Fe–3 wt.% Al powders, all these XRD techniques yielded reasonable, consistent grain size results. However, discrepancies were found in cold pressed (CP-Fe), hot pressed (HP-Fe–3 wt.% Al), and hot isostatically pressed (HIP-Fe–3 wt.% Al) samples. TEM imaging revealed the presence of a certain density of dislocations inside the grains in the HP-Fe–3 wt.% Al and HIP-Fe–3 wt.% Al, which is thought to be partly or fully responsible for the observed discrepancies.

Z-contrast imaging and electron energy-loss spectroscopy with a spatial resolution at the atomic scale provide evidence of an atomically abrupt Si-SiO2, interface. Th micrographs revealed no indication for an interface layer of crystalline oxide at this thermally grown interface. Theoretical ab-initio calculations of two different interface structures showed that even in the most ideal interface the local density of states extends into the region of the oxide band gap. The O-K energy-loss near-edge structure was simulated for both interface models. The comparison of theoretical and experimental results of the O-K near-edge structure agreed and showed that states below the conduction band of the oxide are caused by the dimer-like SiO-Si bridges present in all structural models.
