Music performance anxiety (MPA) is one of the most common disorders among professional musicians, nevertheless, little is known about the disease. With this systematic review, prevalence, risk factors and treatment procedures for MPA were assessed, and for the first time, quality assessments were carried out for all studies using standardized assessment tools. A systematic literature search was conducted via search algorithms in the databases MEDLINE, EMBASE, CINAHL, PsycArticles, PsycInfo and ERIC. Included were case reports, case–control, cohort, cross-sectional and intervention studies examining professional musicians with MPA. For quality assessment, adapted tools of the National Heart, Lung, and Blood Institute were used. A total of 43 studies were included (10 case reports, 21 intervention, 11 cross-sectional, one cohort study). Quality ratings ranged from −11 to 6 out of a maximum of 15/16 points for cross-sectional/cohort studies and −4 to 11 out of 18 points for intervention studies. The prevalence of MPA was between 16.5% and 60%. More women than men were affected and musicians older than 45–50 years reported less MPA than younger musicians. Regarding treatment cognitive behavioural therapy (CBT) and β-blockers were most often researched with beneficial results for CBT. However, studies with adequate control groups for CBT interventions are needed to clarify its efficacy. Studies showed methodological weaknesses, especially in the selection of participants, recording of influencing factors, blinding of interventions, randomization of participants and analysis of comorbidity. Recommendations for further research are made.
Motivated by the problem of jet–flap interaction noise, we study the tonal dynamics that occurs when an isothermal turbulent jet grazes a sharp edge. We perform hydrodynamic and acoustic pressure measurements to characterise the tones as a function of Mach number and streamwise edge position. The observed distribution of spectral peaks cannot be explained using the usual edge-tone model, in which resonance is underpinned by coupling between downstream-travelling Kelvin–Helmholtz wavepackets and upstream-travelling sound waves. We show, rather, that the strongest tones are due to coupling between Kelvin–Helmholtz wavepackets and a family of trapped, upstream-travelling acoustic modes in the potential core, recently studied by Towne et al. (J. Fluid Mech. vol. 825, 2017) and Schmidt et al. (J. Fluid Mech. vol. 825, 2017). We also study the band-limited nature of the resonance, showing the high-frequency cutoff to be due to the frequency dependence of the upstream-travelling waves. Specifically, at high Mach number, these modes become evanescent above a certain frequency, whereas at low Mach number they become progressively trapped with increasing frequency, which inhibits their reflection in the nozzle plane.
To investigate the effects of the nozzle-exit conditions on jet flow and sound fields, large-eddy simulations of an isothermal Mach 0.9 jet issued from a convergent-straight nozzle are performed at a diameter-based Reynolds number of
. The simulations feature near-wall adaptive mesh refinement, synthetic turbulence and wall modelling inside the nozzle. This leads to fully turbulent nozzle-exit boundary layers and results in significant improvements for the flow field and sound predictions compared with those obtained from the typical approach based on laminar flow in the nozzle. The far-field pressure spectra for the turbulent jet match companion experimental measurements, which use a boundary-layer trip to ensure a turbulent nozzle-exit boundary layer to within 0.5 dB for all relevant angles and frequencies. By contrast, the initially laminar jet results in greater high-frequency noise. For both initially laminar and turbulent jets, decomposition of the radiated noise into azimuthal Fourier modes is performed, and the results show similar azimuthal characteristics for the two jets. The axisymmetric mode is the dominant source of sound at the peak radiation angles and frequencies. The first three azimuthal modes recover more than 97 % of the total acoustic energy at these angles and more than 65 % (i.e. error less than 2 dB) for all angles. For the main azimuthal modes, linear stability analysis of the near-nozzle mean-velocity profiles is conducted in both jets. The analysis suggests that the differences in radiated noise between the initially laminar and turbulent jets are related to the differences in growth rate of the Kelvin–Helmholtz mode in the near-nozzle region.
Advance care planning (ACP) increases quality of life and satisfaction with care for those with cancer and their families, yet these important conversations often do not occur. Barriers include patients’ and families’ emotional responses to cancer, such as anxiety and sadness, which can lead to avoidance of discussing illness-related topics such as ACP. Interventions that address psychological barriers to ACP are needed. The purpose of this study was to explore the effects of a mindfulness intervention designed to cultivate patient and caregiver emotional and relational capacity to respond to the challenges of cancer with greater ease, potentially decreasing psychological barriers to ACP and enhancing ACP engagement.
The Mindfully Optimizing Delivery of End-of-Life (MODEL) Care intervention provided 12 hours of experiential training to two cohorts of six to seven adults with advanced-stage cancer and their family caregivers (n = 13 dyads). Training included mindfulness practices, mindful communication skills development, and information about ACP. Patient and caregiver experiences of the MODEL Care program were assessed using semistructured interviews administered immediately postintervention and open-ended survey questions delivered immediately and at 4 weeks postintervention. Responses were analyzed using qualitative methods.
Four salient themes were identified. Patients and caregivers reported the intervention (1) enhanced adaptive coping practices, (2) lowered emotional reactivity, (3) strengthened relationships, and (4) improved communication, including communication about their disease.
Significance of results
The MODEL Care intervention enhanced patient and caregiver capacity to respond to the emotional challenges that often accompany advanced cancer and decreased patient and caregiver psychological barriers to ACP.
Many people with mental illness do not seek professional help. Beliefs about the causes of their current health problem seem relevant for initiating treatment. Our aim was to find out to what extent the perceived causes of current untreated mental health problems determine whether a person considers herself/himself as having a mental illness, perceives need for professional help and plans to seek help in the near future.
In a cross-sectional study, we examined 207 untreated persons with a depressive syndrome, all fulfilling criteria for a current mental illness as confirmed with a structured diagnostic interview (Mini International Neuropsychiatric Interview). The sample was recruited in the community using adverts, flyers and social media. We elicited causal explanations for the present problem, depression literacy, self-identification as having a mental illness, perceived need for professional help, help-seeking intentions, severity of depressive symptoms (Patient Health Questionnaire – Depression), and whether respondents had previously sought mental healthcare.
Most participants fulfilled diagnostic criteria for a mood disorder (n = 181, 87.4%) and/or neurotic, stress-related and somatoform disorders (n = 120, 58.0%) according to the ICD-10. N = 94 (45.4%) participants had never received mental health treatment previously. Exploratory factor analysis of a list of 25 different causal explanations resulted in five factors: biomedical causes, person-related causes, childhood trauma, current stress and unhealthy behaviour. Attributing the present problem to biomedical causes, person-related causes, childhood trauma and stress were all associated with stronger self-identification as having a mental illness. In persons who had never received mental health treatment previously, attribution to biomedical causes was related to greater perceived need and stronger help-seeking intentions. In those with treatment experience, lower attribution to person-related causes and stress were related to greater perceived need for professional help.
While several causal explanations are associated with self-identification as having a mental illness, only biomedical attributions seem to be related to increase perceived need and help-seeking intentions, especially in individuals with no treatment experiences. Longitudinal studies investigating causal beliefs and help-seeking are needed to find out how causal attributions guide help-seeking behaviour. From this study it seems possible that portraying professional mental health treatment as not being restricted to biomedical problems would contribute to closing the treatment gap for mental disorders.
The discovery of the first electromagnetic counterpart to a gravitational wave signal has generated follow-up observations by over 50 facilities world-wide, ushering in the new era of multi-messenger astronomy. In this paper, we present follow-up observations of the gravitational wave event GW170817 and its electromagnetic counterpart SSS17a/DLT17ck (IAU label AT2017gfo) by 14 Australian telescopes and partner observatories as part of Australian-based and Australian-led research programs. We report early- to late-time multi-wavelength observations, including optical imaging and spectroscopy, mid-infrared imaging, radio imaging, and searches for fast radio bursts. Our optical spectra reveal that the transient source emission cooled from approximately 6 400 K to 2 100 K over a 7-d period and produced no significant optical emission lines. The spectral profiles, cooling rate, and photometric light curves are consistent with the expected outburst and subsequent processes of a binary neutron star merger. Star formation in the host galaxy probably ceased at least a Gyr ago, although there is evidence for a galaxy merger. Binary pulsars with short (100 Myr) decay times are therefore unlikely progenitors, but pulsars like PSR B1534+12 with its 2.7 Gyr coalescence time could produce such a merger. The displacement (~2.2 kpc) of the binary star system from the centre of the main galaxy is not unusual for stars in the host galaxy or stars originating in the merging galaxy, and therefore any constraints on the kick velocity imparted to the progenitor are poor.
Perinatal and later postnatal adversities have been shown to adversely affect socioeconomic trajectories, while enhanced early cognitive abilities improve them. However, little is known about the combined influence of these exposures on social mobility. In this study, we examined if childhood IQ moderated the association between four different types of postnatal adversity (childhood socioeconomic disadvantage, childhood sexual abuse, lifetime psychiatric disorder, and trait neuroticism) and annual earnings at 30–35 years of age in a sample of 88 extremely low birth weight survivors. Our results suggested that higher childhood IQ was associated with greater personal income at age 30–35. Extremely low birth weight survivors who did not face psychological adversities and who had higher childhood IQ reported higher income in adulthood. However, those who faced psychological adversity and had higher childhood IQ generally reported lower income in adulthood. Our findings suggest that cognitive reserve may not protect preterm survivors against the complex web of risk factors affecting their later socioeconomic attainment.
The growing global demand for soybeans due to its different uses and by-products, as well as its use in the diet of several livestock species, is forcing the industry to seek alternative protein sources. Environmental concerns related to huge volumes of poultry manure serve as a warning for the choice of more sustainable production systems. Thus, the current review investigates the processing of an insect meal as an alternative protein source to feed broilers. The five desirable features in the selection of insect species proposed by the Food and Agriculture Organization (FAO) show that the high productivity of biomass, feed conversion efficiency and the organic waste conversion capacity from poultry farms is aided by the use of larvae from housefly and black soldier fly species. Brazilian production of such insects is possible due to climatic and environmental conditions and has the potential to supply part of the protein demands of the Brazilian poultry industry when raised in controlled environments. The prevalence of tropical climatic conditions and the possible use of organic waste from poultry farms as substrate can mitigate some environmental issues as well as generate income to smallholder farms prevailing in this activity.
Wavepackets obtained as solutions of the flow equations linearised around the mean flow have been shown in recent work to yield good agreement, in terms of amplitude and phase, with those educed from turbulent jets. Compelling agreement has been demonstrated, for the axisymmetric and first helical mode, up to Strouhal numbers close to unity. We here extend the range of validity of wavepacket models to Strouhal number
and azimuthal wavenumber
by comparing solutions of the parabolised stability equations with a well-validated large-eddy simulation of a Mach 0.9 turbulent jet. The results show that the near-nozzle dynamics can be correctly described by the homogeneous linear model, the initial growth rates being accurately predicted for the entire range of frequencies and azimuthal wavenumbers considered. Similarly to the lower-frequency wavepackets reported prior to this work, the high-frequency linear waves deviate from the data downstream of their stabilisation locations, which move progressively upstream as the frequency increases.
Detailed measurements of crystal outlines and fabrics have been performed on 35 000 crystals in fifteen 10 × 20 cm2 vertical thin sections from the North Greenland Icecore Project (NorthGRIP) ice core, evenly distributed in the depth interval 115–880m. The crystals exhibit important changes over this period. As the ice gets older the mean crystal area increases towards a constant value, the shape of the crystals becomes increasingly irregular, and the area distribution of crystals develops from a single log-normal distribution into a bimodal lognormal distribution. The c-axis fabric of the ice shows a smooth development of an increasingly stronger vertical fabric with depth, and the formation of a weak vertical girdle. Already in the younger samples the fabric is rather strongly oriented towards vertical. The fabric and the area of individual crystals are found not to correlate. A simple model, which takes into account the vertical strain of the ice, is applied in an attempt to determine the crystal growth rate at NorthGRIP.
OBJECTIVES/SPECIFIC AIMS: We hypothesize that both NGF and TNF-α contribute to oral cancer pain by upregulating pro-nociceptive inflammatory cytokines. METHODS/STUDY POPULATION: In total, 48 oral cancer patients were evaluated and their pain scores were measured using a validated oral cancer pain questionnaire. Presence of perineural invasion (PNI) was identified from patients’ pathology reports. We utilized The NIH Cancer Genome Atlas (TCGA) Head and Neck Cancer cohort to investigate the association between pain and genes related to NGF, TNF-α, and their receptors (TRKA, P75, TNF-α receptor 1, and TNF-α receptor 2) in oral cancer samples by employing PNI as a surrogate for pain. Demographic characteristics, clinical characteristics, and genes were analyzed using logistic regression models. A xenograft cancer pain model was created by inoculating human oral cancer cells (HSC-3) into the mouse hind paw. Mice (n=6 per group) were treated with anti-NGF alone, anti-TNF-α alone, a combination of anti-NGF and anti-TNF-α, or PBS (vehicle control). Nociceptive behaviors were measured using an electronic paw withdrawal assay. Paw volume was measured using a plethysmometer. Cytokines in the paw tissues were measured using a multiplex assay kit with 28 cytokines. RESULTS/ANTICIPATED RESULTS: Oral cancer patients with PNI report significantly more pain compared with patients without PNI in our patient cohort (p<0.05). From analysis of TCGA data, we found that PNI is significantly associated with lymphovascular invasion, pathological nodal invasion, and pathological tumor staging (all p<0.05). In adjusted models, we observed that the NGF receptor p75NTR (NGFR) and the TNF-α receptor 1 (TNFRSF1A) were associated with PNI (both p<0.05) and significantly correlated to each other (r=0.25, p<0.001). High levels of TNF-α were present in HSC-3 cell lines and the mouse xenograft cancers. In mice with cancer pain, combined treatment with anti-NGF and anti-TNF-α together provided more effective pain control compared with either anti-NGF or anti-TNF-α treatment alone (p<0.05). We found significantly increased levels of MIP3a, IL-1b, IL-2, IL-4, IL-28b, IL-23, IL17a, IL-31, and IL-33 in cancer mice compared with normal mice (all p<0.05). The combination therapy significantly reduced cytokines MIP3a, IL-1b, IL-4, IL-28b, IL-31, and IL-33 (all p<0.05). DISCUSSION/SIGNIFICANCE OF IMPACT: We show that targeting both NGF and TNF-α provides more effective pain relief in an oral cancer model. These results suggest that therapeutic strategies aimed at both pathways could yield improved pain management for oral cancer patients.
Coherent features of a turbulent Mach 0.9, Reynolds number
jet are educed from a high-fidelity large eddy simulation. Besides the well-known Kelvin–Helmholtz instabilities of the shear layer, a new class of trapped acoustic waves is identified in the potential core. A global linear stability analysis based on the turbulent mean flow is conducted. The trapped acoustic waves form branches of discrete eigenvalues in the global spectrum, and the corresponding global modes accurately match the educed structures. Discrete trapped acoustic modes occur in a hierarchy determined by their radial and axial order. A local dispersion relation is constructed from the global modes and found to agree favourably with an empirical dispersion relation educed from the simulation data. The product between direct and adjoint modes is then used to isolate the trapped waves. Under certain conditions, resonance in the form of a beating occurs between trapped acoustic waves of positive and negative group velocities. This resonance explains why the trapped modes are prominently observed in the simulation and as tones in previous experimental studies. In the past, these tones were attributed to external factors. Here, we show that they are an intrinsic feature of high-subsonic jets that can be unambiguously identified by a global linear stability analysis.
The purpose of this paper is to characterize and model waves that are observed within the potential core of subsonic jets and relate them to previously observed tones in the near-nozzle region. The waves are detected in data from a large-eddy simulation of a Mach 0.9 isothermal jet and modelled using parallel and weakly non-parallel linear modal analysis of the Euler equations linearized about the turbulent mean flow, as well as simplified models based on a cylindrical vortex sheet and the acoustic modes of a cylindrical soft duct. In addition to the Kelvin–Helmholtz instability waves, three types of waves with negative phase velocities are identified in the potential core: upstream- and downstream-propagating duct-like acoustic modes that experience the shear layer as a pressure-release surface and are therefore radially confined to the potential core, and upstream-propagating acoustic modes that represent a weak coupling between the jet core and the free stream. The slow streamwise contraction of the potential core imposes a frequency-dependent end condition on the waves that is modelled as the turning points of a weakly non-parallel approximation of the waves. These turning points provide a mechanism by which the upstream- and downstream-travelling waves can interact and exchange energy through reflection and transmission processes. Paired with a second end condition provided by the nozzle, this leads to the possibility of resonance in limited frequency bands that are bound by two saddle points in the complex wavenumber plane. The predicted frequencies closely match the observed tones detected outside of the jet. The vortex-sheet model is then used to systematically explore the Mach number and temperature ratio dependence of the phenomenon. Targets have been developed to measure supersonic radiation transport in aerogel foams using absorption spectroscopy. The target consists of an aerogel foam uniformly doped with either titanium or scandium inserted into an undoped aerogel foam package. This creates a localized doped foam region to provide spatial resolution for the measurement. Development and characterization of the foams is a key challenge in addition to machining and assembling the two foams so they mate without gaps. The foam package is inserted into a beryllium sleeve and mounted on a gold hohlraum. The target is mounted to a holder created using additive manufacturing and mounted on a stalk. The manufacturing of the components, along with assembly and metrology of the target are described here.
The policies of Republican Governor Scott Walker have come to symbolize a resurgent assault on the public sector, and on public employee unions in particular, by the Republican Party. The fact that this is happening in Wisconsin, the state that in the last century was considered the “laboratory of Progressivism,” makes the politics surrounding these policies all the more compelling. In The Politics of Resentment: Rural Consciousness in Wisconsin and the Rise of Scott Walker, Katherine J. Cramer analyzes the “politics of resentment” surrounding these developments. Employing an ethnographic “method of listening,” Cramer furnishes thick description of the political language employed by rural Wisconsinites, and proceeds to develop an interpretive theory of “political resentment” that illuminates the reasons why lower-class citizens so strongly oppose public policies seeking to offset social and economic inequality. The book is important methodologically and politically. We have thus invited a range of social and political scientists to comment on the book as a work of political science and as a diagnosis of the current political moment.
