The current categorical split of mood disorders in bipolar disorders and depressive disorders has recently been questioned. Two highly unstable personality features, i.e. the cyclothymic temperament (CT) and borderline personality disorder (BPD), have been found to be more common in bipolar II (BP-II) disorder than in major depressive disorder (MDD). According to Kraepelin, temperamental instability was the ‘foundation’ of his unitary view of mood disorders.

The aim was to assess the distributions of the number of CT and borderline personality items between BP-II and MDD. Finding no bi-modal distribution (a ‘zone of rarity’) of these items would support a continuity between the two disorders.

Study setting: an outpatient psychiatry private practice. Interviewer: A senior clinical and mood disorder research psychiatrist. Patient population: A consecutive sample of 138 BP-II and 71 MDD remitted outpatients. Assessment instruments: The structured clinical interview for DSM-IV Axis I Disorders-Clinician Version (SCID-CV), the SCID-II Personality Questionnaire for self-assessing borderline personality traits (BPT) by patients, the TEMPS-A for self-assessing CT by patients. Interview methods: Patients were interviewed with the SCID-CV to diagnose BP-II and MDD, and then patients self-assessed the questions of the Personality Questionnaire relative to borderline personality, and the questions of the TEMPS-A relative to CT. As clinically significant distress or impairment of functioning is not assessed by the SCID-II Personality Questionnaire, a diagnosis of BPD could not be made, but BPT could be assessed (i.e. all BPD items but not the impairment criterion). The distribution of the number of CT and BPT items was studied by Kernel density estimate.

CT and BPT items were significantly more common in BP-II versus MDD. The Kernel density estimate distributions of the number of CT and BPT items in the entire sample had a normal-like shape (i.e. no bi-modality).

The expected finding, on the basis of previous studies and of the present sample features, was a clustering of CT and BPT items on the BP-II side of the curves. Instead, no bi-modality was present in the distributions of the number of CT and BPT items in the entire sample, showing a normal-like shape. By using the bi-modality approach, a continuity between BP-II and MDD seems supported, questioning the current categorical splitting of BP-II and MDD based on classic diagnostic validators.

As psychotic agitated depression is now a well-described form of mixed state during the course of bipolar I disorder, we sought to investigate the diagnostic validity of a new definition for agitated (mixed) depression in bipolar II (BP-II) and major depressive disorder (MDD).

Three hundred and thirty six consecutive outpatients presenting with major depressive episodes (MDE) but without history of mania were evaluated with the Structured Clinical Interview for DSM-IV when presenting for the treatment of MDE. On the basis of history of hypomania they were assigned to BP-II (n = 206) vs. MDD (n = 130). All patients were also examined for hypomania during the current MDE. Mixed depression was operationally defined by the coexistence of a MDE and at least two of the following excitatory signs and symptoms as described by Koukopoulos and Koukopoulos (Koukopoulos A, Koukopoulos A. Agitated depression as a mixed state and the problem of melancholia. In: Akiskal HS, editor. Bipolarity: beyond classic mania. Psychiatr Clin North Am 1999;22:547–64): inner psychic tension (irritability), psychomotor agitation, and racing/crowded thoughts. The validity of mixed depression was investigated by documenting its association with BP-II disorder and with external variables distinguishing it from unipolar MDD (i.e., younger age at onset, greater recurrence, and family history of bipolar disorders). We analyzed the data with multivariate regression (STATA 7).

MDE plus psychic tension (irritability) and agitation accounted for 15.4%, and MDE plus agitation and crowded thoughts for 15.1%. The highest rate of mixed depression (38.6%) was achieved with a definition combining MDE with psychic tension (irritability) and crowded thoughts: 23.0% of these belonged to MDD and 76.9% to BP-II. Moreover, any of these permutations of signs and symptoms defining mixed depression was significantly and strongly associated with external validators for bipolarity. The mixed irritable-agitated syndrome depression with racing-crowded thoughts was further characterized by distractibility (74–82%) and increased talkativeness (25–42%); of expansive behaviors from the criteria B list for hypomania, only risk taking occurred with some frequency (15–17%).

These findings support the inclusion of outpatient-agitated depressions within the bipolar spectrum. Agitated depression is validated herein as a dysphorically excited form of melancholia, which should tip clinicians to think of such a patient belonging to or arising from a bipolar substrate. Our data support the Kraepelinian position on this matter, but regrettably this is contrary to current ICD-10 and DSM-IV conventions. Cross-sectional symptomatologic hints to bipolarity in this mixed/agitated depressive syndrome are virtually absent in that such patients do not appear to display the typical euphoric/expansive characteristics of hypomania—even though history of such behavior may be elicited by skillful interviewing for BP-II. We submit that the application of this diagnostic entity in outpatient practice would be of considerable clinical value, given the frequency with which these patients are encountered in such practice and the extent to which their misdiagnosis as unipolar MDD could lead to antidepressant monotherapy, thereby aggravating it in the absence of more appropriate treatment with mood stabilizers and/or atypical antipsychotics.

Previous reports have demonstrated that depressive mixed state (DMX) (major depressive episode + 3 or more co-occurring intradepressive hypomanic symptoms) and agitated depression are overlapping conditions. The aim of the current study was to examine the relation of DMX and suicide attempt.

Using a structured interview (modified Mini International Neuropsychiatric Interview) and determining all the symptoms of 16 Axis I psychiatric diagnoses defined by the DSM-IV, the authors examined 100 consecutive nonviolent suicide attempters (aged 18-65) within 24 hours after their attempts. Results. DMX was present in 63.0% in the total sample and in 71% among the 89 depressive suicide attempters. More than 90% of the patients with DMX had the symptoms of irritability, distractibility and psychomotor agitation. The rate of DMX was significantly higher among the 29 bipolar (I+II) than in 37 unipolar depressive suicide attempters (90% vs 62%).

This study included suicide attempters who had presented self-poisoning, but not individuals with very high risk of fatality.

In suicide attempters there is a very high prevalence of DMX, especially among bipolar depressive suicide attempters. This study underlines the importance of detecting and appropriate treating DMX and especially depressive bipolar mixed subgroup in suicide behaviour prevention.

La división categórica actual de los trastornos de estado de ánimo en trastornos bipolares y trastornos depresivos se ha puesto en duda recientemente. Se ha encontrado que dos rasgos de personalidad sumamente inestables, es decir, el temperamento ciclotímico (TC) y el trastorno límite de la personalidad (TLP) son más comunes en el trastorno bipolar II (TBP-II) que en el trastorno depresivo mayor (TDM). Según Kraepelin, la inestabilidad temperamental era el “fundamento” de su visión unitaria de los trastornos del estado de ánimo.

El propósito era evaluar las distribuciones del número de elementos de TC y de personalidad límite entre el TBP-II y el TDM. No encontrar una distribución bimodal (una “zona de infrecuencia”) de estos elementos apoyaría una continuidad entre los dos trastornos.

Entorno del estudio: un consultorio privado ambulatorio de psiquiatría. Entrevistador: un psiquiatra clínico de gran experiencia e investigador del trastorno del estado de ánimo. Población de pacientes: Una muestra consecutiva de 138 pacientes ambulatorios con TBP-II y 71 con TDM en remisión. Instrumentos de evaluación: La entrevista clínica estructurada para trastornos del Eje I del DSMIV, versión del Profesional Clínico (SCID-CV), el Cuestionario de Personalidad SCID-II para autoevaluación de rasgos de personalidad límite (RPL) por los pacientes y el TEMPS-A para autoevaluación del TC por los pacientes. Métodos de entrevista: Se entrevistó a los pacientes con la SCID-CV para diagnosticar el TBP-II y el TDM, y luego los pacientes autoevaluaron las preguntas del Cuestionario de Personalidad en relación con la personalidad límite, y las preguntas de TEMPS-A en relación con el TC. Como el Cuestionario de Personalidad SCID-II no evalúa el malestar clínicamente significativo o el deterioro del funcionamiento, no se pudo hacer un diagnóstico de TLP, pero sí evaluar los RPL (es decir, todos los elementos del TLP pero no el criterio de deterioro). La distribución del número de elementos de TC y de RPL se estudió por la estimación de densidad de Kernel.

Los elementos de TC y de RPL eran significativamente más comunes en el TBP-II frente al TDM. Las distribuciones de la estimación de densidad de Kernel del número de elementos de TC y de RPL en la muestra entera tenían una forma análoga a la normal (es decir, no había bimodalidad).

El hallazgo esperado a partir de estudios anteriores y de los rasgos de la presente muestra era un agrupamiento de elementos de TC y de RPL en el lado de TBP-II de las curvas. En cambio, no se puso de manifiesto bimodalidad en las distribuciones del número de elementos de TC y de RPL en la muestra entera, que mostró una forma análoga a la normal. Utilizando el enfoque de la bimodalidad, parece recibir apoyo una continuidad entre el TBP-II y el TDM, lo que pone en duda la división categórica actual de TBP-II y TDM a partir de los validadores diagnósticos clásicos.

– Como la depresión agitada psicótica es ahora una forma bien descrita de estado mixto durante el curso del trastorno bipolar I, intentamos investigar la validez diagnóstica de una nueva definición para la depresión (mixta) agitada en el trastorno bipolar II (TB II) y el trastorno depresivo mayor (TDM).

– Se evaluó a 336 pacientes ambulatorios consecutivos que acudieron con episodio depresivo mayor (EDM) pero sin antecedentes de manía con la Entrevista Clínica Estructurada para el DSM-IV cuando asistían para el tratamiento del EDM. Según los antecedentes de hipomanía se les asignó a TB II (n = 206) frente a TDM (n = 130). Se examinó también a todos los pacientes para hipo manía durante el EDM actual. La depresión mixta se definió operacionalmente por la coexistencia de un EDM y al menos dos de los sig- nos y si'ntomas excitatorios siguientes descritos por Koukopoulos y Koukopoulos (Koukopoulos, A. y Koukopoulos, A., "Agitated depression as a mixed State and the problem of melancolía". En: Akiskal H. S., compilador. "Bipolarity: Beyond classic mania". Psychiatr. Clin.North Am., 1999, 22: 547-564): tensión psíquica interna (irritabilidad), agitación psicomotriz y aceleración/agolpamiento de los pensa-mientos. La validez de la depresión mixta se investigó documentando su asociación con el TB II y con las variables externas que la dis- tinguían del TDM unipolar (es decir, edad menor en el comienzo, mayor recurrencia y antecedentes familiares de trastornos bipolares). Analizamos los datos con regresión multivariable (STATA 7).

– El EDM m ás la tensión psíquica (irritabilidad) y la agitación explicaban el 15,4%, y el EDM más la agitación y el agol- pamiento de los pensamientos, el 15,1%. La tasa más alta de depresión mixta (38,6%) se logró con una definición que combinaba el EDM con la tensión psíquica (irritabilidad) y el agolpamiento de los pensamientos: el 23,0% de estos pertenecía al TDM y el 76,9% al TB II. Además, cualquiera de estas permutaciones de los signos y si'ntomas que definían la depresión mixta se asociaba significativamente y con fuerza con validadores externos para la bipolaridad. El síndrome depresivo irritable-agitado mixto con aceleración-agolpamiento de los pensamientos se caracterizaba además por la distraibilidad (74-82%) y el aumento de la locuacidad (25-42%); de las conductas expansi- vas de la lista B de criterios para hipomanía, sólo la adopción de riesgos se produjo con alguna frecuencia (15-17%).

– Estos hallazgos apoyan la inclusión de las depresiones ambulatorias agitadas dentro del espectro bipolar. La depresión agitada se valida aquí como una forma disfóricamente excitada de la melancolía, lo que debería inclinar a los profesionales clínicos a pen- sar que un paciente de esta índole pertenece a un sustrato bipolar o deriva de él. Nuestros datos apoyan la posición de Kraepelin sobre este asunto, pero lamentablemente esto es contrario a las convenciones actuales de la CIE-10 y el DSM-IV. Las indicaciones sintomato- lógicas transversales hacia la bipolaridad en este síndrome depresivo mixto/agitado están casi ausentes, ya que estos pacientes no pare- cen mostrar las características eufóricas/expansivas típicas de la hipomanía, aun cuando una entrevista hábil para el TB II pueda obtener antecedentes de esta conducta. Reconocemos que la aplicación de esta entidad diagnóstica en la consulta ambulatoria sería de valor clí-nico considerable, dada la frecuencia con la que se encuentra a estos pacientes en ella y el grado en el que su diagnóstico erróneo como TDM unipolar podría llevar a monoterapia antidepresiva, agravándola así en ausencia de tratamiento más apropiado con estabilizadores del estado de ánimo, antipsicóticos atípicos, o ambos.
