Human infection with antimicrobial-resistant Campylobacter species is an important public health concern due to the potentially increased severity of illness and risk of death. Our objective was to synthesise the knowledge of factors associated with human infections with antimicrobial-resistant strains of Campylobacter. This scoping review followed systematic methods, including a protocol developed a priori. Comprehensive literature searches were developed in consultation with a research librarian and performed in five primary and three grey literature databases. Criteria for inclusion were analytical and English-language publications investigating human infections with an antimicrobial-resistant (macrolides, tetracyclines, fluoroquinolones, and/or quinolones) Campylobacter that reported factors potentially linked with the infection. The primary and secondary screening were completed by two independent reviewers using Distiller SR®. The search identified 8,527 unique articles and included 27 articles in the review. Factors were broadly categorised into animal contact, prior antimicrobial use, participant characteristics, food consumption and handling, travel, underlying health conditions, and water consumption/exposure. Important factors linked to an increased risk of infection with a fluoroquinolone-resistant strain included foreign travel and prior antimicrobial use. Identifying consistent risk factors was challenging due to the heterogeneity of results, inconsistent analysis, and the lack of data in low- and middle-income countries, highlighting the need for future research.
The following position statement from the Union of the European Phoniatricians, updated on 25th May 2020 (superseding the previous statement issued on 21st April 2020), contains a series of recommendations for phoniatricians and ENT surgeons who provide and/or run voice, swallowing, speech and language, or paediatric audiology services.
Objectives
This material specifically aims to inform clinical practices in countries where clinics and operating theatres are reopening for elective work. It endeavours to present a current European view in relation to common procedures, many of which fall under the aegis of aerosol generating procedures.
Conclusion
As evidence continues to build, some of the recommended practices will undoubtedly evolve, but it is hoped that the updated position statement will offer clinicians precepts on safe clinical practice.
Waterfall Bluff is a rock shelter in eastern Pondoland, South Africa, adjacent to a narrow continental shelf that limited coastline movements across glacial/interglacial cycles. The archaeological deposits are characterized by well-preserved stratigraphy, faunal, and botanical remains alongside abundant stone artifacts and other materials. A comprehensive dating protocol consisting of 5 optically stimulated luminescence ages and 51 accelerator mass spectrometry 14C ages shows that the record of hunter-gatherer occupations at Waterfall Bluff persisted from the late Pleistocene to the Holocene, spanning the last glacial maximum and the transition from the Pleistocene to the Holocene. Here, we provide detailed descriptions about the sedimentary sequence, chronology, and characteristics of the archaeological deposits at Waterfall Bluff. Remains of marine mollusks and marine fish also show, for the first time, that coastal foraging was a component of some hunter-gatherer groups’ subsistence practices during glacial phases in the late Pleistocene. The presence of marine fish and shellfish further demonstrates that hunter-gatherers selectively targeted coastal resources from intertidal and estuarine habitats. Our results therefore underscore the idea that Pondoland's coastline remained a stable and predictable point on the landscape over the last glacial/interglacial transition being well positioned for hunter-gatherers to access resources from the nearby coastline, narrow continental shelf, and inland areas.
Social implications and long-term consequences of mental disorders regarding sick days at work, unemployment rates and early retirement are substantial. There is no clear understanding
1) whether and which social psychiatric strategies of vocational rehabilitation and
2) whether and how atypical antipsychotics are successful in supporting vocational rehabilitation in schizophrenia.
Objectives
Primary objective was to demonstrate the enhanced ability of schizophrenic patients to be successfully in employment after attending specific “job-consulting” programs. Secondary objective was to demonstrate positive effects of ziprasidone and risperidone on work-related functions.
Aims
Aim of the study was to enlighten the complex relationship between working performance of schizophrenic patients, their social psychiatric and medical treatment, and their cognitive capabilities in a first step.
Methods
The study was conducted in inpatients, day-clinic- or outpatients diagnosed with schizophrenia, paranoid type (ICD 10: F20.0). All patients received regular pharmacological treatment and intense cognitive training, additionally one group completed specific “job-consulting” programs. ANT, CPT, WCST, PANSS and WBN were performed after 3, 6, 12 and 24 months.
Results
At present eighteen schizophrenic patients (6 female and 12 male, mean age: 33.6 years) have been included in the study. Analyzing the data after 3 months the results are:
1) specific “job-consulting” programs increase the probability of employment
2) both, ziprasidone and risperidone, exert positive and comparable effects in work-related functions,
3) the better the cognitive capability the better the vocational outcome.
Conclusions
Specific “job-consulting” programs constitute a clinically relevant therapeutic option in vocational rehabilitation of schizophrenic patients.
Animal studies have suggested that exposure of the middle ear to topical local anaesthesia may be ototoxic. This study aimed to report sensorineural hearing outcomes and patients’ satisfaction in those who underwent myringotomy and ventilation tube insertion using topical local anaesthesia.
Methods
Twenty-nine patients (32 ears) were operated on. Pre- and post-operative audiology findings were compared. A Likert-type questionnaire on treatment satisfaction was completed at the end of the procedure.
Results
Median patient age was 55 years (range, 27–88 years). Pre- and post-operative bone conduction pure tone averages were 26.76 dB and 25.26 dB respectively (mean reduction of −1.22 dB, 95 per cent confidence interval of −5.91 to 8.13 dB; p = 0.7538). One ear (3 per cent) had a reduction in pure tone average of 10 dB.
Conclusion
The results suggest that sensorineural hearing loss is not a complication of ear exposure to topical local anaesthesia during myringotomy and ventilation tube insertion. The procedure was well perceived.
The spatial-intensity profile of light reflected during the interaction of an intense laser pulse with a microstructured target is investigated experimentally and the potential to apply this as a diagnostic of the interaction physics is explored numerically. Diffraction and speckle patterns are measured in the specularly reflected light in the cases of targets with regular groove and needle-like structures, respectively, highlighting the potential to use this as a diagnostic of the evolving plasma surface. It is shown, via ray-tracing and numerical modelling, that for a laser focal spot diameter smaller than the periodicity of the target structure, the reflected light patterns can potentially be used to diagnose the degree of plasma expansion, and by extension the local plasma temperature, at the focus of the intense laser light. The reflected patterns could also be used to diagnose the size of the laser focal spot during a high-intensity interaction when using a regular structure with known spacing.
Two and a half decades ago, the discovery of Mercury’s polar deposits from Earth-based radar observations provided the first tantalizing, but limited, evidence for the possibility of water ice on the solar system’s innermost planet. Identifying the materials in Mercury’s polar deposits was one of the six major science questions that originally motivated the MESSENGER mission. In the course of the mission’s more than four Earth years of operations in orbit about Mercury, MESSENGER produced multiple datasets to investigate Mercury’s polar deposits: determinations of regions of permanent shadow, neutron spectrometer observations, laser altimeter reflectance measurements, thermal model results, and direct images of the deposits. These datasets provided compelling evidence that in addition to substantial amounts of water ice stored in Mercury’s polar deposits, there are also other volatile materials, postulated to be dark, organic-rich compounds that bury the water ice deposits. This chapter reviews MESSENGER’s investigations of Mercury’s polar deposits and discusses the resulting implications for the origin and evolution of Mercury’s polar water ice.
We develop scholarship on status in international politics by focusing on the social dimension of small and middle power status politics. This vantage opens a new window on the widely-discussed strategies social actors may use to maintain and enhance their status, showing how social creativity, mobility, and competition can all be system-supporting under some conditions. We extract lessons for other thorny issues in status research, notably questions concerning when, if ever, status is a good in itself; whether it must be a positional good; and how states measure it.
The stable-isotopic signature of dissolved inorganic carbon (DIC) has been routinely used in temperate lake systems to investigate the biogeochemical dynamics of carbon. We studied seven perennially ice-covered lakes in the McMurdo Dry Valleys, Antarctica, to ascertain how carbon cycling affects the δ13C of DIC in water columns of these systems. Unlike temperate lakes and, in fact, most polar lake systems, the permanent ice covers of these lakes eliminate physical mixing (turnover) and hence redistribution of DIC in the lakes, as well as minimize CO2 exchange with the atmosphere. These important and unique physical constraints have significant impact on carbon dynamics in the lakes, and important consequences for the δ13C distribution. The geochemistry in these lakes is influenced in varying amounts by landscape position, hydrologic input and their evolutionary history. Five of these lakes (both lobes of Lake Bonney, and Lakes Fryxell, Miers and Vanda) have surface water δ13C ratios of 0–4‰, Lake Hoare has more negative values, while Lake Joyce, the highest-elevation lake, has a much higher value (10.5‰). All of the lakes have upper- to mid-depth δ13C maxima reflecting biological uptake of 12C. Only four of the lakes (Lakes Vanda, Joyce, Hoare and Fryxell) have deep waters with negative values of δ13C, implying rigorous remineralization of 12C at depth. Lake Miers, the only lake that is not closed basin, has the smallest δ13C variation with depth, indicating that hydrologic exchange greatly influences the δ13C signal.
Snow sublimation is a fundamental process that affects the snow crystal structure and is important for ice-core interpretation, remote sensing, snow hydrology and chemical processes in snow. Prior studies have shown that sublimation can change the isotopic content of the remaining snow; these studies have inferred sublimation rates using field data, and were unable to control many of the environmental parameters that determine sublimation rate (e.g. temperature, relative humidity, snow microstructure). We present sublimation rate measurements on snow samples in the laboratory, where we have controlled many of these parameters simultaneously. We use the same experimental apparatus to determine sublimation rate, investigate the isotopic effects of sublimation, and study the isotopic exchange between vapor and solid. Our results suggest that pore spaces in snow are almost always at saturation vapor pressure; undersaturation may be possible in large pore spaces or in regions of rapid interstitial airflow. We present a revised formulation for determining the mass-transfer coefficient for snow as a linear function of Reynolds number (hm = 0.566Re + 0.075), estimate the fractionation coefficient for sublimating snow, and provide evidence for isotopic exchange between vapor and solid.
We derive mass changes of the Greenland ice sheet (GIS) for 2003–07 from ICESat laser altimetry and compare them with results for 1992–2002 from ERS radar and airborne laser altimetry. The GIS continued to grow inland and thin at the margins during 2003–07, but surface melting and accelerated flow significantly increased the marginal thinning compared with the 1990s. The net balance changed from a small loss of 7 ± 3 Gt a−1 in the 1990s to 171 ± 4 Gt a−1 for 2003–07, contributing 0.5 mm a−1 to recent global sea-level rise. We divide the derived mass changes into two components: (1) from changes in melting and ice dynamics and (2) from changes in precipitation and accumulation rate. We use our firn compaction model to calculate the elevation changes driven by changes in both temperature and accumulation rate and to calculate the appropriate density to convert the accumulation-driven changes to mass changes. Increased losses from melting and ice dynamics (17–206 Gt a−1) are over seven times larger than increased gains from precipitation (10–35 Gt a−1) during a warming period of ∼2 K (10 a)−1 over the GIS. Above 2000 m elevation, the rate of gain decreased from 44 to 28 Gt a−1, while below 2000 m the rate of loss increased from 51 to 198 Gt a−1. Enhanced thinning below the equilibrium line on outlet glaciers indicates that increased melting has a significant impact on outlet glaciers, as well as accelerating ice flow. Increased thinning at higher elevations appears to be induced by dynamic coupling to thinning at the margins on decadal timescales.
Ice deformation and basal motion characterize the dynamical behavior of the Greenland ice sheet (GrIS). We evaluate the contribution of basal motion from ice deformation measurements in boreholes drilled to the bed at two sites in the western marginal zone of the GrIS. We find a sustained high amount of basal motion contribution to surface velocity of 44–73% in winter, and up to 90% in summer. Measured ice deformation rates show an unexpected variation with depth that can be explained with the help of an ice-flow model as a consequence of stress transfer from slippery to sticky areas. This effect necessitates the use of high-order ice-flow models, not only in regions of fast-flowing ice streams but in all temperate-based areas of the GrIS. The agreement between modeled and measured deformation rates confirms that the recommended values of the temperature-dependent flow rate factor A are a good choice for ice-sheet models.
The British Society of Audiology has produced clear guidelines as to how otoscopy should be undertaken; however, no nationally recognised guidelines exist for the wider clinical community. Images of otoscopy appear in many books, journals, magazines and websites.
Objective:
This study aimed to determine the rate of non-compliance with good practice in images of otoscopy, the seriousness of the breach, and whether this is more common in sites for professionals or the general public.
Method:
Google Images was searched using the terms ‘otoscopy’ and ‘ear examination’. A total of 200 images were identified and collated. The images were reviewed for compliance with good practice standards.
Results:
Only 12.75 per cent of the images were graded as having no breach of good practice standards.
Conclusion:
Professional websites have a responsibility to show best practice. When choosing an image, the source of the image needs to be carefully considered.
As in the past, the primary activity of the IAU Working Group on Cartographic Coordinates and Rotational Elements has been to prepare and publish a triennial (“2009”) report containing current recommendations for models for Solar System bodies (Archinal et al. (2011a)). The authors are B. A. Archinal, M. F. A'Hearn, E. Bowell, A. Conrad, G. J. Consolmagno, R. Courtin, T. Fukushima, D. Hestroffer, J. L. Hilton, G. A. Krasinsky, G. Neumann, J. Oberst, P. K. Seidelmann, P. Stooke, D. J. Tholen, P. C. Thomas, and I. P. Williams. An erratum to the “2006” and “2009” reports has also been published (Archinal et al. (2011b)). Below we briefly summarize the contents of the 2009 report, a plan to consider requests for new recommendations more often than every three years, three general recommendations by the WG to the planetary community, other WG activities, and plans for our next report.
We examine the dynamic relationships between relief spending and local private labor markets using a panel data set of relief, private employment, and private earnings. Positive shocks to relief during the First New Deal were followed by increased private employment and earnings, consistent with demand stimulus in that period. On the other hand, increases in work relief spending during the Second New Deal were followed by decreased employment and increased earnings, consistent with crowding out. The timing of spending is consistent with claims that the Roosevelt administration used relief spending to sway elections.
