OBJECTIVES/GOALS: Community-based residency programs often lack formal training in research scholarship required by ACGME. To address this need, UCSF’s CTSI collaborated with residency leaders to implement a self-paced online curriculum for residents called Training in Practice Based Research (TIPR). We describe characteristics of the initial trainee cohort. METHODS/STUDY POPULATION: In the 2022-23 academic year, TIPR was offered to 10 UCSF-affiliated family medicine residency programs across Northern California and the Central Valley, and 8 chose to participate. An additional community-based psychiatry residency independently contacted our team and was also granted permission to participate. We conducted baseline surveys with participants to understand their prior research experience and motivation to join TIPR. Descriptive data for demographics of trainees and their prior research experience were collected using Qualtrics. Thematic analyses were conducted on qualitative responses. RESULTS/ANTICIPATED RESULTS: Of 32 participants, 29 completed the survey (91%). Learners identified as 40% non-Hispanic White, 28% Asian, 16% Hispanic, 9% non-Hispanic Black, and 15% non-Hispanic other. 28% were motivated to participate in the program because it was a residency requirement, 31% wanted to improve their scholarly skills and confidence, 16% were interested in career development, and 6% were interested in networking. 19% reported no research experience. Participants are currently working on scholarly projects designed during the first year of TIPR. In 2023-2024, with the addition of two new family medicine residency programs, an additional 40 residents have enrolled in TIPR. In April 2024, we will present data on projects completed, and demographics of the full cohort. DISCUSSION/SIGNIFICANCE: With CTSI support, TIPR has reached a large cohort of ethnically diverse physician trainees in community-based settings. Future evaluation will focus on whether TIPR increases the quantity and quality of practice-based research within residency training programs served by this program.
Traditionally, research institutions have valued individual achievements such as principal investigator and lead authorship status as primary indicators in the academic promotions process. However, the scientific process increasingly requires collaboration by teams of researchers across multiple disciplines, sometimes including experts outside academia, often referred to as “team science.” We sought to determine whether there is agreement about what constitutes team science at our academic institution and whether current promotion processes sufficiently incentivize faculty participation in team science.
Methods:
We conducted 20 qualitative interviews with academic leaders (N = 24) at the University of California, San Francisco (UCSF) who supervise faculty promotions processes. Participants were asked to share their definitions of team science and the extent to which faculty receive credit for engaging in these activities during the promotions process. A subset of participants also completed a brief survey in which they ranked the importance of participation in team science relative to other factors that are traditionally valued in the promotions process. Interview data were examined by two analysts using structural coding. Descriptive analyses were conducted of survey responses.
Results:
Though team science is valued at UCSF, definitions of team science and the approach to assigning credit for team science in academic promotions processes varied widely. Participants suggested opportunities to bolster support for team science.
Conclusions:
Efforts to define and provide transparent faculty incentives for team science should be prioritized at institutions, like UCSF, seeking to advance faculty engagement in collaborative research.
Infrared (Visible-Near Infrared-Shortwave Infrared (VNIR-SWIR)) spectroscopy is a cost-effective technique for mineral identification in the field. Modern hand-held spectrometers are equipped with on-board spectral libraries that enable rapid, qualitative analysis of most minerals and facilitate recognition of key alteration minerals for exploration. Spectral libraries can be general or customized for specific mineral deposit environments. To this end, careful collection of spectra in a controlled environment on pure specimens of key minerals was completed using the National Mineral Reference Collection (NMC) of the Geological Survey of Canada. The spectra collected from specimens in the ‘Kodama Clay Collection’ were processed using spectral plotting software and each new example was validated before being added to a group of spectra considered for incorporation into the on-board library of the handheld ASD-TerraSpec Halo near-infrared (NIR) mineral identification instrument. Spectra from an additional suite of mineral samples of the NMC containing REE, U, Th, and/or Nb are being prepared for a new, publicly available spectral library. These minerals commonly occur in carbonatite or alkali intrusive deposits, and as such will assist in the exploration for critical metals.
This chapter explores terrorism, terrorist groups, and potential solutions to terrorism. It discusses conceptual and legal definitions of terrorism. It then provides historical context, describing terrorist waves driven by ideologies such as anarchism, anti-colonialism, New Left perspectives, and various religions. The chapter looks at who becomes a terrorist, taking account of theories of grievance, radicalization, recruitment by terrorist groups, lone wolf terrorism, and foreign fighter recruitment. It considers relationships between regime type and terrorism, examining why democracies, anocracies, and autocracies respectively might be more or less likely to attract terrorist violence, and exploring issues such as media coverage, grievance, and repression. Terrorist tactics and how these might diffuse across groups are discussed, as are terrorist group cooperation and state sponsorship of terrorism. The chapter also looks at the challenges of crafting effective counterterrorism policy, with a focus on deradicalization. It then applies many of these concepts to a quantitative study on whether targeting terrorist group leaders helps stop terrorism, and a case study of al-Qaeda.
This study investigated the association between early extubation (EE) and the degree of postoperative intensive care unit (ICU) support after the Fontan procedure, specifically evaluating the volume of postoperative intravenous fluid (IVF) and vasoactive-inotropic score (VIS).
Methods:
Retrospective analysis of patients who underwent Fontan palliation from 2008 to 2018 at a single center was completed. Patients were initially divided into pre-institutional initiative towards EE (control) and post-initiative (modern) cohorts. Differences between the cohorts were assessed using t-test, Wilcoxon, or chi-Square. Following stratification by early or late extubation, four groups were compared via ANOVA or Kruskal-Wallis Test.
Results:
There was a significant difference in the rate of EE between the control and modern cohorts (mean 42.6 versus 75.7%, p = 0.01). The modern cohort demonstrated lower median VIS (5 versus 8, p = 0.002), but higher total mean IVF (101±42 versus 82 ±27 cc/kg, p < 0.001) versus control cohort. Late extubated (LE) patients in the modern cohort had the highest VIS and IVF requirements. This group received 67% more IVF (140 ± 53 versus 84 ± 26 cc/kg, p < 0.001) and had a higher median VIS at 24 hours (10 (IQR, 5–10) versus 4 (IQR, 2–7), p < 0.001) versus all other groups. In comparison, all EE patients had a 5-point lower median VIS when compared to LE patients (3 versus 8, p= 0.001).
Conclusions:
EE following the Fontan procedure is associated with reduced post-operative VIS. LE patients in the modern cohort received more IVF, potentially identifying a high-risk subgroup of Fontan patients deserving of further investigation.
Chapter 5 offers the first systemic examination of the strategic considerations that underpin an emerging trend that has not yet gained enough attention in either academic or policy circles – the growing role of counterterrorism in China’s foreign policy. China needs to enhance its force’s counterterrorism capabilities, protect the growing number of Chinese nationals and assets abroad, and build an image as a responsible international stakeholder. However, these goals conflict with China’s desire to minimize grievances arising from its economic activities, which could lead to the country becoming a target for international terrorist groups. Empirical analyses of original data on the counterterrorism joint military exercises held by China and foreign forces indicate that China is highly cautious and selective when it comes to these exercises. Military counterterrorism cooperation tends to closely follow Chinese economic investments.
Chapter 4 documents and analyzes China’s domestic policies aimed at countering Uyghur violence. We discuss the broad securitization of Xinjiang, including budgets and the forces involved. Drawing on the best available data on Uyghur-related political violence and China’s public security expenditure in Xinjiang, we present the first rigorous assessment of the feedback loop of violence and repression in Xinjiang. We demonstrate that government repression is not systematically followed by increased Uyghur violence and that increased security expenditures are excessive and inefficient, especially in the long run. This chapter also traces the recent strategic shift in China’s policies from postattack securitization toward actively and forcibly promoting ethnic mingling and “de-extremification.” While this policy reorientation has been attributed to Beijing’s intolerance of instability, our analysis shows that it is a result of a more complex set of competing priorities within the Chinese government.
Chapter 1 sets the stage for our study, introducing both the political violence and repression endemic to Xinjiang. We provide a brief account of the recent history of Xinjiang and the Uyghurs to contextualize the current situation before introducing the book’s motivations, arguments, and structure.
