We would like to correct an error that was printed in the above mentioned article: p 711: The sentence ‘Ionized calcium levels in those three cases were 1.01, 1.09, and 1.11mg/dl respectively.’ should have read: ‘Ionized calcium levels in those three cases were 1.01, 1.09, and 1.11mmol/l respectively.’
