In a previous work the Authors have reported that the so-called «Klippel-Feil Syndrome» should in many cases be ascribed to a hereditary disease of the axial skeleton. This disease can express itself in various degrees (macroforms or microforms) fundamentally identified by fissure and synostosis: the Authors suggested that the disease might be identified as «schisosynostosis axialis congenita familiaris».

In the present work the Authors report the case of a twin suffering from congenital osseous torticollis, whose co-twin, apparently healthy, was found instead to be affected by posterior synostosis of C2 and C3 and by fissure of S1 and S2, as well as bv sacralization of L5 and traces of cervical platyspondyly.

In the same family the father was found to suffer from basal segmentation of the dens epistrophei, by cervical kyphosis and fissure of S1; a brother was affected by synostosis of the laminae of C2 and C3.

In the discussion of the case the Authors come to the conclusion that congenital osseous torticollis can be ascribed (as was the case with Klippel-Feil's Syndrome) to «schisosynostosis axialis congenita familiaris» of which it represents another possible anatomo-clinical expression.

RIASSUNTO

In un precedente lavoro gli AA. hanno dimostrato come la cosidetta «sindrome di Klippel-Feil» debba essere riferita in molti casi ad una malattia ereditaria dello scheletro assiale che può assumere espressioni diverse — macroforme e microforme — essenzialmente caratterizzate da schisi e sinostosi: per questa malattia gli AA. hanno proposto il termine di schisosinostosi assiale congenita familiare.

Nel presente lavoro gli AA. descrivono il caso di un gemello affetto da torcicollo osseo congenito, il cui cogemello, apparentemente sano, risultò invece colpito da sinostosi degli archi posteriori di C2 e C3 e schisi di S1 e S2, oltre a sacralizzazione di L5 e note di platispondilia cervicale. Nell'ambito familiare il padre risultò affetto da segmentazione basale del dente dell'epistrofeo, cifosi cervicale e schisi di S1; un fratello da sinostosi delle lamine di C2 e C3.

Dalla discussione del proprio caso gli AA. concludono che il torcicollo congenito di origine scheletrica, al pari della sindrome di Klippel-Feil, può essere ricondotto alla schisosinostosi assiale congenita familiare, di cui rappresenta anch'esso una possibile espressione anatomo-clinica.

RÉSUMÉ

Dans une étude précédante les Auteurs ont démontré qu'en plusieurs cas ce qu'on appelle «syndrôme de Klippel-Feil» doit être rapporté à une maladie héréditaire du squelette axial. Cette maladie peut s'exprimer de différentes façons (macroformes et microformes) qui se caractérisent essentiellement par fissure et Synostose: les Auteurs ont proposé d'appeler cette maladie «schisosynostosis axialis congenita familiaris».

Dans cette étude les Auteurs décrivent le cas d'un jumeau qui souffre de torticolis osseux congénital, dont le co-jumeau, apparemment sain, est atteint au contraire de synostose des arcs postérieur de C2 et C3, déhiscence des arcs de S1 et S2 sacralisation de L5 et traces de platyspondylie cervicale.

On a constaté que, dans la même famille, le père présentait une segmentation basale du dens epistrophei, cyphose cervicale et fissure des lames de C2 et C3.

Dans la discussion de ce cas les Auteurs sont arrivés à la conclusion que le torticolis osseux congénital peut être attribué, de même que la syndrôme de Klippel-Feil, à «schisosynostosis axialis congenita familiaris», dont elle représente une autre expression anatomo-clinique.

ZUSAMMENFASSUNG

In einer vorhergehenden Arbeit bewiesen die Verfasser, dass das sogenannte «Klippel-Feil' sche Syndrom» in vielen Fällen mit einer Erbkrankheit des Achsenskeletts in Zusammenhang zu bringen ist, die verschiedenen Ausdruck — Makroformen und Mikroformen — annehmen kann. Diese zeichnen sich hauptsächlich durch Spaltungen und Synostosen aus. Die Verfasser schlugen für diese Krankheit die Bezeichnung «Schisosynostosis axialis congenita familiaris» vor.

In der vorliegenden Arbeit besprechen die Verfasser den Fall eines Zwillings mit angeborenem osteogenem Schiefhals, wärend bei dem anscheinend gesunden Paarling Synostosen der hinteren Bögen C2 und C3, Spaltung von S1 und S2 sowie Sakralisation von L5 mit Anzeichen von Platyspondylosis cervicalis zu bemerken sind. Bei der Sippenuntersuchung ergibt sich beim Vater eine basale Segmentation des Zahns des Epistropheus, Halskyphose und Spaltung von S1; bei einem Bruder Synostosen der laminae von C2 und C3 Aus der Besprechung ihres Falles schliessen die Verfasser, dass sich der angeborene osteogene Schiefhals — genau wie das Klippel-Feil'sche Syndrom — auf die Schisosynostosis axialis congenita familiaris zurückführen lässt, von der er ebenfalls eine mögliche anatomisch-klinische Form darstellt.
